Head trauma

A week or two ago, I was playfully hit in the back of the head while messing around with friends.
Initially, I went to the hospital to see a doctor, who said a CT scan wasn't necessary and that I should just monitor my condition.
I felt a bit better at first, but after exercising yesterday, I suddenly started feeling dizzy and unwell.
Should I go back for another check-up?

Dr. Zhang Junwei reply Neurosurgery

Headaches and dizziness are common after a concussion, so it is important to rest after the injury.
Overexertion or excessive physical activity can only exacerbate the symptoms of the concussion.

After experiencing a head injury, especially one that involves a blow to the back of the head, it is crucial to monitor your symptoms closely. Given your recent experience of feeling dizzy and unwell after initially improving, it is advisable to seek further evaluation. Here’s why:
1. Symptoms of Concern: Dizziness, especially if it appears after a period of improvement, can be a sign of complications such as a concussion or even intracranial bleeding. While many head injuries do not result in serious issues, symptoms like persistent headaches, dizziness, nausea, or changes in consciousness warrant further investigation.
2. Delayed Symptoms: It’s important to note that some symptoms may not appear immediately after the injury. For instance, intracranial bleeding can occur days after the initial trauma. This is known as a delayed hemorrhage, which can lead to increased pressure in the skull and potentially serious complications if not addressed promptly.

3. Medical Evaluation: Since you initially saw a doctor who recommended observation, it’s essential to follow up if your condition changes. A healthcare professional may recommend imaging studies, such as a CT scan, to rule out any serious issues. Even if the initial assessment did not indicate the need for a CT scan, your new symptoms could change that recommendation.

4. Concussion Management: If you are diagnosed with a concussion, management typically involves physical and cognitive rest. Gradually returning to normal activities is essential, but this should be done under medical supervision. If you experience worsening symptoms, it’s crucial to avoid activities that could risk further injury.

5. When to Seek Help: You should seek immediate medical attention if you experience any of the following:
- Severe or worsening headache
- Repeated vomiting
- Confusion or difficulty concentrating
- Weakness or numbness in any part of your body
- Seizures
- Loss of consciousness
6. Follow-Up Care: Even if your symptoms seem mild, it’s wise to have a follow-up appointment with a healthcare provider. They can assess your symptoms, provide guidance on recovery, and determine if any further testing is necessary.

In summary, given your recent symptoms of dizziness and discomfort after a head injury, it is prudent to return to a healthcare provider for further evaluation. Early detection and management of potential complications can significantly improve outcomes and ensure your safety. Always prioritize your health and err on the side of caution when it comes to head injuries.
