Fell while playing sports and hit the back of the head?

Hello Doctor, I fell while playing basketball on November 28 and hit the back of my head.
I sat on the floor for a moment and then got up.
There was no bleeding, but I had a swollen bump.
After 3 to 4 hours at home, I felt uneasy and went to the emergency room for a CT scan, which came back normal, so I went home and applied ice.
The next day, I had a slight loss of appetite and some mild nausea, so I went to see a neurosurgeon.
Is there any further testing I should undergo to feel more at ease?

Dr. Lai Zhaokang reply Neurosurgery

Hello, you have undergone a CT scan which showed no intracranial hemorrhage, indicating that there is generally no immediate life threat.
Your symptoms are likely similar to a concussion.
Please rest and take care of yourself.
Typically, medication treatment in the neurosurgery outpatient clinic will show improvement in about two weeks.
Currently, there is no need for further examinations.
Please return for a follow-up appointment for evaluation and monitoring by the neurosurgeon.
Wishing you well, Dr.

When it comes to head injuries, especially those sustained during sports like basketball, it's crucial to be vigilant about symptoms and seek appropriate medical evaluation when necessary. Based on your description, you experienced a fall that resulted in a bump on the back of your head, and although you initially felt fine, you later developed some concerning symptoms such as decreased appetite and mild nausea.
First and foremost, it's good to hear that you sought immediate medical attention after the injury and underwent a CT scan, which came back normal. This is an important step, as CT scans can help rule out serious conditions such as intracranial bleeding or fractures. However, even with a normal scan, it's essential to monitor for any changes in your condition.

Here are some key points to consider regarding your situation:
1. Symptoms to Monitor: After a head injury, it's important to watch for specific symptoms that may indicate a more serious issue. These include:
- Persistent or worsening headache
- Nausea or vomiting
- Dizziness or balance issues
- Confusion or difficulty concentrating
- Changes in vision or hearing
- Seizures
- Loss of consciousness
If you experience any of these symptoms, it would be prudent to seek further medical evaluation.

2. Post-Concussion Syndrome: It's possible that you may be experiencing symptoms related to post-concussion syndrome, which can occur after a mild traumatic brain injury. Symptoms can include headaches, dizziness, fatigue, and cognitive difficulties that may persist for weeks or even months after the initial injury. If your symptoms do not improve or worsen, follow-up with a neurologist or a specialist in concussion management may be beneficial.

3. Rest and Recovery: After a head injury, rest is crucial. This includes both physical and cognitive rest. Avoiding strenuous activities, including sports, and limiting activities that require intense concentration (like studying or screen time) can help facilitate recovery. Gradually reintroducing activities as tolerated is important, but listen to your body and avoid pushing yourself too hard.

4. Follow-Up Care: Since you have already seen a neurologist, it may be helpful to schedule a follow-up appointment to discuss your ongoing symptoms. They may recommend additional tests or therapies, such as vestibular rehabilitation if you are experiencing balance issues or cognitive therapy if you have trouble concentrating.

5. When to Seek Immediate Care: If at any point you feel that your symptoms are worsening or if you develop new symptoms, do not hesitate to return to the emergency department. It's better to err on the side of caution when it comes to head injuries.

In summary, while your initial evaluation was reassuring, continued monitoring of your symptoms is essential. Ensure you rest adequately and consult with your healthcare provider if your symptoms persist or worsen. Keeping an open line of communication with your medical team will help ensure that you receive the best care possible as you recover from your head injury.
