Abnormal sensation in the ribs?
Hello doctor, I've been feeling some discomfort under my rib cage these past few days.
There's no difficulty in breathing, but I'm wondering if it could be due to doing sit-ups.
I had a pneumothorax surgery a few years ago, and I'm worried about a possible recurrence.

Dr. Yang Wenda reply Pulmonology
Hello: It is possible that the muscle pain is caused by doing sit-ups.
If there is any shortness of breath, a chest X-ray should be performed.

Experiencing unusual sensations in the rib area can be concerning, especially for someone with a history of thoracic surgery, such as a previous pneumothorax (collapsed lung) operation. It’s important to approach this situation with a thorough understanding of potential causes and the appropriate steps to take.
First, let’s consider the nature of the sensation you’re experiencing. If it’s a mild discomfort or a feeling of tightness without any accompanying symptoms like difficulty breathing, it may be related to muscle strain, especially if you’ve recently engaged in activities like sit-ups or other exercises that involve the core muscles. Muscle strains can occur when the muscles are overworked or improperly used, leading to discomfort that can sometimes be mistaken for more serious conditions.
However, given your medical history, it’s prudent to be cautious. A previous pneumothorax can lead to concerns about lung health, and while it’s unlikely that a muscle strain would cause a recurrence of a pneumothorax, any unusual sensations in the chest area should be evaluated. Symptoms such as sharp pain, shortness of breath, or a feeling of pressure in the chest would warrant immediate medical attention.
In addition to muscle strain, other potential causes for rib discomfort could include:
1. Costochondritis: This is an inflammation of the cartilage that connects a rib to the sternum. It can cause localized pain that may worsen with certain movements or pressure.
2. Rib Fractures: Although less likely if there was no trauma, stress fractures can occur, particularly in individuals who engage in high-impact activities.
3. Pulmonary Issues: Conditions such as pleuritis (inflammation of the lining of the lungs) or even a recurrence of pneumothorax should be considered, especially if the sensation is accompanied by any respiratory symptoms.
4. Gastrointestinal Issues: Sometimes, discomfort in the rib area can be referred pain from gastrointestinal problems, such as acid reflux or gallbladder issues.
5. Nerve Irritation: Conditions like intercostal neuralgia, where the nerves running between the ribs become irritated, can also lead to unusual sensations.
Given these possibilities, it’s essential to monitor your symptoms closely. If the sensation persists, worsens, or is accompanied by any concerning symptoms such as difficulty breathing, chest pain, or significant changes in your overall health, you should seek medical attention promptly. A healthcare provider may recommend imaging studies, such as a chest X-ray or CT scan, to rule out any serious conditions.
In the meantime, consider avoiding activities that exacerbate the discomfort, and apply heat or ice to the area as needed. Over-the-counter pain relief medications may also help alleviate any discomfort, but consult with a healthcare professional before starting any new medication.
In summary, while muscle strain is a likely cause of your rib discomfort, given your history of thoracic surgery, it’s wise to remain vigilant and consult with a healthcare provider if your symptoms do not improve or if you have any additional concerns. Your health and peace of mind are paramount, and addressing these sensations early can help prevent potential complications.
