Small white spots on the inner sides of the cheeks?
Hello Doctor, two days ago in the morning when I woke up, I noticed that there were some spots on both sides of my tongue, with the right side being more pronounced.
Both sides have some slight appearance towards the throat.
I visited a small clinic, and the doctor said it was an aging phenomenon caused by fat.
After doing some research online, I found that it resembles leukoplakia, but the small white spots are less noticeable.
Additionally, I have a small mouth ulcer that is very minor and does not affect my eating.
I would like to know your opinion, thank you, Doctor!

Dr. Li Jiaxuan reply Otolaryngology
It is recommended to reduce刺激 foods or oral friction for two weeks.
If there is no improvement, it is advised to go to the hospital for a biopsy.
Wishing you well.

The presence of small white spots inside the cheeks can be attributed to several potential causes, and understanding these can help in determining the appropriate course of action. Based on your description, it seems that you have already consulted a physician who suggested that these spots may be related to fat deposits due to aging. However, it is essential to consider other possibilities as well.
1. Fordyce Spots: These are small, painless, pale bumps that can appear on the inner cheeks, lips, or genital area. They are sebaceous glands that are not associated with hair follicles and are completely harmless. They can appear more prominently with age and are often mistaken for other conditions.
2. Oral Candidiasis (Thrush): This is a fungal infection caused by an overgrowth of Candida yeast, which can lead to white patches in the mouth. These patches may be painful and can sometimes bleed if scraped. If you have a compromised immune system or are taking antibiotics, you may be at a higher risk for this condition.
3. Leukoplakia: This condition presents as white patches in the mouth that cannot be scraped off. It can be caused by irritation from tobacco use, alcohol, or ill-fitting dentures. While leukoplakia itself is not cancerous, it can sometimes be a precursor to oral cancer, so it is important to monitor any changes.
4. Lichen Planus: This is an inflammatory condition that can affect the mucous membranes in the mouth, leading to white, lacy patches or spots. It can be associated with discomfort and may require treatment if it causes pain or difficulty eating.
5. Mucosal Fibroma: These are benign tumors that can appear as small white or pink bumps in the mouth. They are usually painless and can be removed if they cause discomfort or cosmetic concerns.
Given that you mentioned having a sore in your mouth, it is possible that the irritation from the sore could be contributing to the appearance of these spots. If the spots are not causing pain or discomfort and do not interfere with eating, they may not require immediate treatment. However, it is crucial to monitor them for any changes in size, color, or sensation.
Recommendations:
- Observation: Keep an eye on the spots for any changes. If they start to grow, change color, or become painful, it would be wise to return to your healthcare provider for further evaluation.
- Maintain Oral Hygiene: Good oral hygiene can help prevent infections and other oral health issues. Brush your teeth twice a day and consider using an antibacterial mouthwash.
- Avoid Irritants: If you smoke or consume alcohol, consider reducing or eliminating these substances, as they can irritate the oral mucosa and exacerbate conditions like leukoplakia.
- Dietary Considerations: Ensure you are consuming a balanced diet rich in vitamins and minerals, particularly vitamin B12, folate, and iron, which are essential for maintaining healthy mucous membranes.
- Follow-Up: If the spots persist or if you have any concerns, follow up with a dental or medical professional who specializes in oral health. They may recommend a biopsy or other diagnostic tests if they suspect a more serious condition.
In summary, while the small white spots inside your cheeks may be benign, it is essential to remain vigilant and consult with healthcare professionals if you notice any changes or if you have concerns about your oral health.
