White membrane on both sides of the urethral opening on the glans?

Hello Doctor, I have noticed a white membrane on the glans, usually on both sides of the urethral opening.
Currently, only a small part in the middle remains a healthy pink color.
There is no pain, itching, or odor, and I do not experience any burning or stinging sensation while urinating.
I visited a urologist, who diagnosed it as a fungal infection and prescribed a topical antifungal ointment.
However, it seems to have had little effect, and the white area appears to have increased.
I have a habit of cleaning daily, and before my visit to the urologist, I used body wash to clean the area.
The doctor specifically advised me not to use any cleansing products in the future.
I continue to wash the area daily, but I use warm water and gently rub it instead of cold water.
I am wondering if it is possible that I am over-cleaning, causing the skin on the glans to regenerate and leading to a misdiagnosis by the doctor.
I am concerned that my gentle rubbing may be creating a vicious cycle.
Due to my job, it takes a long time to get time off to see a doctor again.
I hope you can advise me on the following: 1.
Should I change my cleaning method to using cold water and gently rubbing? 2.
Is there any medication that can soften the white membrane so I can remove it? (I have tried soaking in warm water with no effect.) 3.
Should I continue applying the ointment? 4.
Can a cotton swab with a small amount of iodine be effective in removing it?

Hello: 1) Use warm water to rinse while bathing, do not rub, pat dry, and if there are no symptoms, there is no need to apply ointment.
Wear loose-fitting underwear.
2) A cotton swab with a small amount of iodine can effectively clean? --> If there are no symptoms, it is unnecessary.
Use condoms during sexual activity, and avoid public water places such as swimming pools and hot springs.
Wishing you good health.

The presence of a white membrane around the urethral opening, particularly on the glans penis, can be concerning and may indicate various underlying conditions. Based on your description, it seems you have already consulted a urologist who suggested a fungal infection and prescribed a topical antifungal cream. However, since you have not observed significant improvement, let's explore your questions in detail.

1. Cleaning Method: It is essential to maintain proper hygiene without overdoing it. Over-cleaning, especially with harsh soaps or scrubbing, can irritate the sensitive skin of the glans and potentially lead to a condition known as balanitis, which is inflammation of the glans. Instead of using soap, it is advisable to clean the area gently with warm water. Using ice-cold water may not be necessary and could cause discomfort. The key is to be gentle and avoid any products that could irritate the skin further. If you are currently using any soaps or cleansers, it would be best to stop and stick to plain water.

2. Softening the White Membrane: There are no specific over-the-counter treatments designed to soften a white membrane on the glans. However, soaking the area in warm water may help. If warm water has not been effective, you might consider trying a saline solution (a mixture of salt and water) as it can help cleanse the area and may promote healing. Avoid any aggressive scrubbing or use of cotton swabs that could cause further irritation.

3. Continued Use of Topical Cream: If the antifungal cream prescribed by your doctor has not shown any improvement after a reasonable period (usually a week or two), it is advisable to follow up with your healthcare provider. They may want to reassess the situation, as the white membrane could be due to other causes, such as a different type of infection, irritation, or even a skin condition like lichen sclerosus. Continuing to use the cream without improvement may not be beneficial.

4. Use of Cotton Swabs and Iodine: Using a cotton swab with iodine is not recommended for cleaning sensitive areas like the glans. Iodine can be irritating and may disrupt the natural flora of the skin, potentially worsening the condition. It is best to avoid any harsh chemicals or irritants in this area.

In summary, it is crucial to maintain gentle hygiene practices and avoid over-cleaning. If the white membrane persists or worsens, or if you develop any new symptoms (such as pain, itching, or discharge), it is essential to return to your healthcare provider for further evaluation. They may consider other diagnoses or treatments, including a possible referral to a dermatologist if the condition does not improve with standard treatments. Remember, self-diagnosing and treating can sometimes lead to complications, so professional guidance is always the best course of action.
