Bowel movement issues
Hello, doctor.
I have some concerns about my bowel movements.
Sometimes my stools are very loose, to the point where I can see vegetable remnants.
It's quite exaggerated; for example, after eating dumplings around noon, I can see vegetable pieces in my stool by 9 PM to midnight.
Of course, this happens during episodes of diarrhea.
However, today I did not have diarrhea, and my stool this afternoon had a diameter of about 1.2 to 1.5 cm.
But when I went to the bathroom again in the evening, the diameter was 2 cm, and the shape was somewhat square.
My weight has remained stable, and I have been exercising.
Throughout May, my weight has been consistent during my observations.
I have not experienced black stools or blood in my stools.
However, one day after eating red dragon fruit, I had a bit of diarrhea, and the color of the stool was somewhat pinkish.
I am curious if this indicates rapid digestion.
Previously, I did have well-formed stools (though this does not happen often).
Currently, I do not have dizziness, urgency, or abdominal pain (only during diarrhea), and I have not experienced any weight loss.
Could you please explain what might cause changes in stool shape or provide some recommendations for tests? Thank you, doctor.

Dr. Lin Minghui reply Gastroenterology and Hepatology
Hello: If your weight has not changed and you are exercising, it is possible that you have a fast metabolism.
The shape of your stool is related to the food you consume.
A diet high in fiber typically results in well-formed stools, while a diet high in fatty foods may lead to looser stools.
Consuming colored foods, such as red dragon fruit, can cause your urine and stool to take on that color.
If there are issues with your intestines, such as enteritis or malabsorption, your stools may also be loose.
In cases of colorectal polyps, stools may sometimes appear deformed or elongated.
If you are concerned about intestinal issues, you may consider stool testing or a colonoscopy.
Wishing you safety and health.
Sincerely, Dr.

Understanding bowel issues, particularly irregular stool shape and consistency, can be complex and multifactorial. From your description, it seems you are experiencing variations in stool consistency and shape, which can be influenced by several factors including diet, hydration, and gastrointestinal health.
Firstly, the presence of undigested food particles, such as vegetable remnants in your stool, can indicate that your digestive system is not breaking down food as efficiently as it should. This can occur for various reasons, including rapid transit time through the intestines, which may lead to diarrhea or loose stools. When food moves too quickly through the digestive tract, there may not be enough time for proper digestion and absorption, resulting in the appearance of undigested food in your stool.
The shape and consistency of your stool can also be affected by your diet. For instance, a diet high in fiber can lead to bulkier stools, while a low-fiber diet may result in harder, more compact stools. The fact that you mentioned seeing variations in the diameter of your stool, ranging from 1.2 to 2 cm, suggests that your bowel movements are not consistent, which can be normal but may also indicate some underlying issues.
The square shape of your stool is particularly interesting. While normal stool is typically cylindrical, variations in shape can occur due to several factors, including dehydration, dietary changes, or even the way the stool is formed in the intestines. If the stool is consistently square or has an unusual shape, it may be worth discussing with a healthcare provider, as it could indicate a need for further evaluation.
Regarding the color changes you observed after consuming red dragon fruit, it is not uncommon for certain foods to alter the color of your stool. The pinkish hue you described could be due to the pigments in the fruit, which can sometimes pass through the digestive system without being fully broken down. This is generally not a cause for concern unless accompanied by other symptoms.
Given that you have not experienced significant symptoms such as abdominal pain, weight loss, or blood in your stool, it may not be urgent to seek immediate medical attention. However, if these irregularities persist or worsen, or if you develop new symptoms, it would be advisable to consult a healthcare professional. They may recommend tests such as stool analysis, blood tests, or imaging studies to rule out any underlying gastrointestinal conditions.
In the meantime, consider keeping a food diary to track what you eat and how it correlates with your bowel movements. This can help identify any dietary triggers that may be contributing to your symptoms. Additionally, ensuring adequate hydration and a balanced diet rich in fiber can promote regular bowel function and improve stool consistency.
In summary, while occasional variations in stool shape and consistency can be normal, persistent changes warrant further investigation. Monitoring your symptoms and discussing them with a healthcare provider can help ensure that any potential issues are addressed promptly.
