What is the date of conception?
Doctor, I would like to ask about my last menstrual period, which started around July 30 (I have irregular periods).
I had sexual intercourse on August 30 without a condom, and on September 5, I initially did not use a condom but put one on before ejaculation.
On September 18, I took a pregnancy test and found out I was pregnant (the two lines appeared very clearly).
That day, I went to see a doctor, and the urine test confirmed the pregnancy.
An abdominal ultrasound only showed a suspected black spot, and the doctor estimated that I was about four weeks pregnant, suggesting conception occurred in early September.
On September 23, I went to another clinic for an abdominal ultrasound, where the doctor estimated that I was about three weeks pregnant, with the ultrasound showing a gestational sac (GS) of 0.73 cm and a gestational age (GA) of 5 weeks and 3 days.
Does this mean that the likelihood of conception on August 30 is higher? Additionally, I have been tracking my basal body temperature and recorded a high temperature on the morning of September 4.
Does this indicate that it is less likely I conceived on September 5? I have heard that after a high temperature, the egg should have already been released.
Could you please help me estimate which day I likely conceived? Thank you!
Vivi, 30~39 year old female. Ask Date: 2020/09/27

Hello, two days is too close, and it's possible for both.
It's hard to say for sure which day it is.
Wishing you good health.

Determining the conception date can be particularly challenging for individuals with irregular menstrual cycles. In your case, you mentioned that your last menstrual period started on July 30, and you had unprotected intercourse on August 30 and September 5. You received a positive pregnancy test on September 18, which indicates that conception likely occurred within a specific timeframe.
To estimate your conception date, we can consider a few key points:
1. Ovulation Timing: Generally, ovulation occurs about 14 days before the start of your next menstrual period. However, since your cycles are irregular, pinpointing ovulation can be more complex. If your cycle is typically longer, ovulation may occur later than the standard 14-day mark.
2. Intercourse Dates: You had unprotected intercourse on August 30, which is a crucial date to consider. If ovulation occurred shortly after this date, it is possible that conception could have taken place then. The September 5 encounter, while also significant, may be less likely to result in conception if ovulation had already occurred by that time.
3. Basal Body Temperature (BBT): You mentioned recording a high temperature on September 4. A sustained rise in BBT typically indicates that ovulation has occurred. If you observed a significant temperature increase on September 4, it is likely that ovulation happened around that time, making conception from the August 30 encounter more plausible. After ovulation, the egg is viable for fertilization for about 12 to 24 hours, while sperm can survive in the female reproductive tract for up to five days. Therefore, if you ovulated on September 4, the likelihood of conception from the September 5 encounter would be lower.
4. Ultrasound Findings: The ultrasound results you received on September 23 indicated a gestational age of approximately 5 weeks and 3 days, which aligns with a conception date around early September. However, given the timing of your intercourse and the BBT data, it is reasonable to conclude that conception likely occurred closer to August 30.
In summary, while the ultrasound suggests a conception date of early September, the combination of your intercourse on August 30, the high BBT on September 4, and the nature of sperm viability points towards a higher probability of conception occurring around August 30.
Additional Considerations:
- Irregular Cycles: For individuals with irregular cycles, tracking ovulation through methods such as BBT, ovulation predictor kits, or cervical mucus monitoring can provide more accurate insights into fertile windows.
- Follow-Up Care: It is essential to continue monitoring your pregnancy with your healthcare provider. Early ultrasounds can sometimes yield varying results, so follow-up scans may help clarify gestational age and confirm the health of the pregnancy.
- Pregnancy Symptoms: Pay attention to any symptoms you may experience, and do not hesitate to reach out to your healthcare provider with any concerns.
In conclusion, while the ultrasound suggests a conception date of early September, the evidence points towards a higher likelihood of conception occurring around August 30, especially considering your BBT readings. It is always best to consult with your healthcare provider for personalized advice and to ensure a healthy pregnancy.
