Regarding ejaculation disorders?
Hello doctor, I had sexual intercourse with my girlfriend in the doggy style position while wearing a condom.
During ejaculation, due to tightness, the semen could not be expelled from the urethra and I had to withdraw to release the semen from the vagina.
I did not have this issue before I turned 30.
I have chronic prostatitis; could this be related?

Dr. Li Xinyi reply Urology
Chronic prostatitis may be related to the narrowing and partial obstruction of the ejaculatory duct.
If there are issues with infertility, it is advisable to consult the urology department at a major hospital for examination.

Ejaculation disorders can be a complex issue, often influenced by a variety of physical and psychological factors. Based on your description, it seems that you are experiencing a specific type of ejaculation disorder where you are unable to ejaculate while having penetrative intercourse, particularly when using a condom. This can be frustrating and concerning, especially since you did not have this issue before the age of 30.
One potential cause of your symptoms could indeed be related to your chronic prostatitis. Chronic prostatitis, or chronic pelvic pain syndrome, can lead to various sexual dysfunctions, including difficulties with ejaculation. The inflammation and discomfort associated with prostatitis can affect the normal functioning of the prostate and surrounding structures, which are crucial for ejaculation. This condition can lead to a sensation of tightness or discomfort during intercourse, which may inhibit the ability to ejaculate.
Additionally, the use of condoms can sometimes contribute to ejaculation difficulties. Some men find that condoms create a sensation that is too tight or restrictive, which can interfere with the natural process of ejaculation. If the condom is particularly snug, it may create additional pressure that makes it difficult for semen to be expelled from the urethra. This could explain why you are able to ejaculate when you withdraw and remove the condom, as this alleviates the pressure.
Moreover, psychological factors can also play a significant role in ejaculation disorders. Anxiety about sexual performance, fear of pain, or concerns about your chronic condition can lead to increased tension during intercourse, which may further complicate the ability to ejaculate. It’s important to consider both the physical and psychological aspects of your situation.
To address your concerns, here are some potential solutions and recommendations:
1. Consult a Urologist: Since you have a history of chronic prostatitis, it would be beneficial to consult a urologist who specializes in male sexual health. They can provide a thorough evaluation and determine if your prostatitis is contributing to your ejaculation difficulties.
2. Medication Review: If you are currently taking any medications for prostatitis or other conditions, discuss with your doctor whether these could be affecting your sexual function. Sometimes, medications can have side effects that impact libido or ejaculation.
3. Explore Different Condoms: If the tightness of the condom is an issue, consider trying different brands or types of condoms. There are various sizes and materials available that may provide a better fit and enhance comfort during intercourse.
4. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises such as Kegel exercises may help improve ejaculatory control and overall sexual function. A physical therapist specializing in pelvic health can guide you through these exercises.
5. Psychological Support: If anxiety or psychological factors are contributing to your difficulties, consider speaking with a therapist or counselor who specializes in sexual health. They can help you work through any performance anxiety or emotional concerns you may have.
6. Communication with Your Partner: Open communication with your girlfriend about your experiences and feelings can help alleviate some of the pressure you may feel during sexual encounters. Discussing your concerns together can foster understanding and intimacy.
In conclusion, ejaculation disorders can stem from a combination of physical and psychological factors. Given your history of chronic prostatitis, it is advisable to seek professional medical advice to explore the underlying causes and appropriate treatment options. With the right support and interventions, many men find relief from these issues and can regain confidence in their sexual health.
