Balanitis issues?
Hello Doctor, I previously had balanitis and after seeing a doctor, I was prescribed Meikang cream for application.
My symptoms improved, but recently there has been a recurrence.
The originally red spots have faded, but there are small white bumps in the center, and there are new red spots appearing in other areas.
It hurts a lot when I rinse with water (I’m not sure if it’s because the water pressure is too strong, but I used a saline solution bottle for rinsing), and there was a little bit of bleeding.
Could you please explain what might be causing this? Also, what are the small white bumps? Should I continue applying the medication? I have three questions for you, Doctor.

More Info
Recurrent balanitis, or inflammation of the glans penis, can be a frustrating condition for many men. It is characterized by symptoms such as redness, swelling, and discomfort in the genital area. Understanding the symptoms, causes, and treatment options is crucial for effective management.
Symptoms
The symptoms of recurrent balanitis can vary but typically include:
- Redness and swelling of the glans penis
- Itching or burning sensation
- Pain during urination or when washing the area
- Discharge or unusual odor
- White patches or lesions, which may indicate a fungal infection or other underlying issues
In your case, the presence of white small bumps and increased redness suggests that there may be an ongoing inflammatory process or a secondary infection. The pain during washing, especially if accompanied by bleeding, is concerning and warrants further evaluation.
Causes
Recurrent balanitis can be caused by a variety of factors, including:
1. Infections: Fungal infections (like Candida), bacterial infections, or sexually transmitted infections can lead to inflammation.
2. Irritation: This can be due to soaps, detergents, or other irritants that come into contact with the genital area.
3. Poor hygiene: Inadequate cleaning can lead to the accumulation of smegma, which can irritate the glans.
4. Skin conditions: Conditions like eczema or psoriasis can also manifest as balanitis.
5. Diabetes: Men with diabetes are more prone to infections, including balanitis, due to higher glucose levels in the urine, which can promote fungal growth.
Treatment Options
Given your symptoms and the recurrence of balanitis, here are some treatment options to consider:
1. Topical Treatments: Continue using the prescribed medication (Mei Kang cream) as directed by your physician. This cream is often used to treat fungal infections and may help reduce inflammation. However, if symptoms persist or worsen, you should consult your doctor for a reevaluation.
2. Gentle Hygiene Practices: When washing the area, use mild, unscented soap and warm water. Avoid using high-pressure water streams, as they can cause further irritation. Pat the area dry gently after washing.
3. Avoid Irritants: Identify and avoid any potential irritants, such as scented soaps, lotions, or tight-fitting clothing that may exacerbate the condition.
4. Consult a Specialist: If the symptoms do not improve with topical treatments, or if you notice any new symptoms (like increased pain, discharge, or fever), it is essential to see a urologist or dermatologist. They may perform tests to rule out infections or other underlying conditions.
5. Consider Systemic Treatments: In some cases, oral antifungal or antibiotic medications may be necessary if a more severe infection is present.
Conclusion
Recurrent balanitis can be a complex condition with various underlying causes. The white bumps you are experiencing could be indicative of a fungal infection or another dermatological issue, and the bleeding during washing is a sign that you should seek medical attention. Continuing the prescribed treatment while maintaining gentle hygiene practices is essential, but do not hesitate to reach out to your healthcare provider for further evaluation and management. Early intervention can help prevent complications and improve your quality of life.
