Symptoms of myocarditis?
Dear Doctor,
I am currently studying abroad and it is not convenient for me to seek medical attention.
I apologize for asking, but I tested positive for COVID-19 14 days ago and have not experienced any symptoms, so I have been isolating at home.
However, in the last two to three days, I have suddenly felt chest pain that lasts for about less than a second.
The pain is located approximately one centimeter to the left of the sternum, along the line connecting to the left clavicle.
The exact point of pain varies each time, but it feels like heart pain.
I have read about the effects of COVID-19 and myocarditis, and I would like to ask you, does this presentation resemble myocarditis? If so, what tests should I undergo? Thank you very much for your response!
Hu, 30~39 year old female. Ask Date: 2020/10/16
Dr. Wu Xueming reply Cardiology
Your symptoms do not appear to be indicative of myocarditis.
However, if the symptoms persist and worsen, it is advisable to go to the hospital for further evaluation.

Understanding myocarditis symptoms after a COVID-19 infection is crucial, especially given the ongoing concerns about the long-term effects of the virus. Myocarditis, which is inflammation of the heart muscle, can occur as a complication of viral infections, including COVID-19. However, it is important to differentiate between various types of chest pain and their potential causes.
Based on your description, the sudden chest pain you are experiencing, which lasts for less than a second and occurs in a specific location, may not necessarily indicate myocarditis. Myocarditis typically presents with more persistent symptoms, such as prolonged chest pain, shortness of breath, fatigue, or palpitations. The pain associated with myocarditis is often more diffuse and can be accompanied by other systemic symptoms, such as fever or malaise.
Given that you have recently recovered from COVID-19 and are experiencing these symptoms, it is understandable to be concerned. However, the transient nature of your chest pain, especially if it is not associated with other symptoms like shortness of breath, dizziness, or significant fatigue, may suggest a less serious issue. It could be related to anxiety, muscle strain, or even gastrointestinal discomfort, which can mimic cardiac symptoms.
If you are worried about myocarditis or if your symptoms persist or worsen, it is advisable to seek medical attention. While you mentioned that you are currently studying abroad and find it difficult to seek in-person medical care, there are options available. Many healthcare providers offer telemedicine services, which can allow you to consult with a physician remotely. They can assess your symptoms more thoroughly and determine if further evaluation is necessary.
In terms of diagnostic tests, if myocarditis is suspected, a healthcare provider may recommend several investigations, including:
1. Electrocardiogram (ECG): This test measures the electrical activity of your heart and can help identify any irregularities that may suggest myocarditis.
2. Echocardiogram: An ultrasound of the heart can assess heart function and structure, looking for signs of inflammation or damage.
3. Blood tests: These can check for markers of inflammation, cardiac enzymes, and other indicators that might suggest heart muscle injury.
4. Cardiac MRI: In some cases, this imaging test can provide detailed information about the heart muscle and help confirm a diagnosis of myocarditis.
5. Chest X-ray: This can help rule out other causes of chest pain, such as pneumonia or other lung issues.
In conclusion, while your symptoms warrant attention, they may not necessarily indicate myocarditis. It is essential to monitor your symptoms closely and seek medical advice if they persist or if you develop new symptoms. Engaging with a healthcare provider through telemedicine could be a practical first step in addressing your concerns and ensuring your health and well-being during your studies abroad. Remember to prioritize your health and seek help when needed.
