Stasis of blood?

Hello Dr.
Lee, about a month ago, I fell while riding in a car due to the driver making an emergency stop.
I sustained an abrasion on my lower leg (approximately 4-5 cm) which healed in about a week, but the bruising next to it has not improved.
It doesn't hurt unless I touch it, but there is a hard lump that is very painful when pressed.
The area surrounding the lump is also tender.
Should I continue to monitor this, or do I need to see a doctor? If I need to see a doctor, should I make an appointment with a surgeon?

Sandy, 60~69 year old female. Ask Date: 2020/10/19

Dr. Li Zhonghuai reply Surgery

Hello! The external wound has healed, and the blood clot inside will gradually be absorbed.
If there is a concurrent infection, debridement will be necessary.
It is recommended to consult a general surgeon.
Take care! Changhua Hospital cares about you!

Persistent bruising and the presence of a hard lump following an injury can be concerning, especially if the symptoms persist for an extended period. Based on your description, it seems that you experienced a significant impact to your leg, resulting in a bruise and a hard lump that is painful to touch. Here’s a detailed overview of what might be happening and when you should seek medical advice.

Understanding the Symptoms
1. Bruising: Bruising occurs when small blood vessels (capillaries) break under the skin due to trauma, causing blood to leak into the surrounding tissues. This can lead to discoloration and swelling. While bruises typically heal within a few weeks, persistent bruising can indicate underlying issues.

2. Hard Lump: The hard lump you are feeling could be a hematoma, which is a localized collection of blood outside of blood vessels. Hematomas can occur when blood pools in the tissues after an injury. They may feel firm or hard and can be painful, especially if they are pressing on surrounding tissues or nerves.

3. Pain: The pain you experience when touching or pressing on the lump suggests that there may still be inflammation or irritation in the area. This is not uncommon after an injury, but it should gradually improve over time.

When to Seek Medical Advice
You should consider seeking medical attention if you experience any of the following:
- Persistent Symptoms: If the hard lump and pain do not improve over the next few days or weeks, it is advisable to consult a healthcare professional.
- Increasing Pain: If the pain worsens or becomes unbearable, this could indicate complications such as infection or a more serious injury.

- Changes in Color or Temperature: If the area around the bruise or lump becomes increasingly red, warm, or swollen, this could be a sign of infection.

- Mobility Issues: If you find it difficult to move your leg or if the pain interferes with your daily activities, you should seek medical advice.

- New Symptoms: If you develop new symptoms such as fever, chills, or systemic signs of infection, you should seek immediate medical attention.

Which Specialist to See
In your case, it would be appropriate to see a general surgeon or an orthopedic surgeon. Both specialists are equipped to evaluate soft tissue injuries, including hematomas and other complications that may arise from trauma. They can perform a physical examination and may recommend imaging studies (like an ultrasound or MRI) to assess the extent of the injury and determine the best course of action.

Conclusion
While it’s common for bruises and soft tissue injuries to take time to heal, persistent symptoms warrant further evaluation. It’s better to err on the side of caution, especially if you are experiencing pain and have a hard lump that has not resolved. Consulting with a healthcare professional will provide you with peace of mind and ensure that any potential complications are addressed promptly. Remember to keep track of your symptoms and communicate any changes to your doctor during your visit.
