Numbness, itching, and tingling in the hands and feet?
Initially, there was acute urticaria with widespread hives, and patches appeared on the palms and soles that were very itchy.
After taking medication prescribed by the hospital for urticaria, the hives subsided, but after two days, the palms and soles still felt swollen and itchy with a prickling sensation.
What could be the issue? Which specialty should I consult, and what treatments should be considered? Thank you.

Hello Ms.
Hsia: You mentioned that the hives have subsided, but after a couple of days, your hands and feet are still swollen and experiencing itching and tingling.
This could be a delayed allergic reaction causing sensory abnormalities.
I recommend that you visit a rheumatology and immunology clinic for further evaluation and management.

Numbness and itching in the hands and feet can be distressing symptoms that may arise from various underlying conditions. Given your history of acute urticaria (hives), it is important to consider both allergic reactions and other potential causes for your current symptoms.
Firstly, the initial episode of hives suggests that your body may have had an allergic reaction, which can sometimes lead to residual symptoms even after the primary rash has resolved. The lingering sensations of swelling, itching, and tingling in your hands and feet could be attributed to several factors:
1. Post-Inflammatory Changes: After an allergic reaction, the skin and underlying tissues can remain sensitive and reactive. This can lead to prolonged itching and a feeling of swelling, even after the visible rash has subsided.
2. Nerve Involvement: Itching and numbness can also indicate nerve irritation or damage. Conditions such as peripheral neuropathy, which can be caused by diabetes, vitamin deficiencies, or other systemic issues, may manifest as tingling, numbness, or a burning sensation in the extremities.
3. Histamine Release: Even after the hives have resolved, histamine and other inflammatory mediators can remain elevated in the body, leading to ongoing symptoms. This can be exacerbated by stress, temperature changes, or exposure to allergens.
4. Circulatory Issues: Swelling and numbness can also be related to circulatory problems, where blood flow to the extremities is compromised. This can happen due to various reasons, including prolonged sitting or standing, or vascular conditions.
5. Dermatological Conditions: Other skin conditions, such as eczema or contact dermatitis, could also cause itching and swelling in the hands and feet, especially if there was exposure to irritants or allergens.
Given these possibilities, it is advisable to consult with a healthcare provider who specializes in dermatology or allergy and immunology. They can perform a thorough examination and may recommend tests such as blood work to check for underlying conditions, skin tests for allergies, or nerve conduction studies if neuropathy is suspected.
In terms of treatment, the approach will depend on the underlying cause identified:
- Antihistamines: If the symptoms are still related to an allergic response, antihistamines can help alleviate itching and swelling.
- Topical Treatments: Corticosteroid creams or ointments may be prescribed to reduce inflammation and itching on the skin.
- Lifestyle Modifications: Identifying and avoiding triggers that may exacerbate your symptoms is crucial. Keeping a symptom diary can help pinpoint potential allergens or irritants.
- Physical Therapy: If nerve involvement is confirmed, physical therapy may be beneficial to improve nerve function and alleviate symptoms.
In conclusion, while your symptoms may be a continuation of an allergic reaction, it is essential to explore other potential causes. Consulting with a specialist will provide clarity and guide you toward the most effective treatment plan. Remember to monitor your symptoms closely and seek immediate medical attention if you experience severe swelling, difficulty breathing, or other alarming symptoms.
