Pearl-like papules on the foreskin?

Recently, I noticed a small white bump on my penile foreskin that is neither painful nor itchy.
I later squeezed it and expelled a white substance.
Every time I squeeze it, the same spot reappears in about 1-2 weeks.
I did some research online and found that it could possibly be herpes on the foreskin, but I haven't had sexual intercourse with a female partner for almost two years.
Could this be the case?

Niming, 40~49 year old female. Ask Date: 2020/10/23

Dr. Du Shixi reply Urology

Hello: The herpes is of the vesicular type, and there should not be any substance present.
If you are concerned, you may seek medical examination.
Wishing you good health.

Understanding white bumps on the foreskin can be concerning, especially when they appear to be persistent and recurrent. Based on your description, it sounds like you are experiencing a condition that is not uncommon among males. The white bumps you are noticing could be several things, but they are unlikely to be herpes, especially given your history of not engaging in sexual activity for nearly two years.

One common cause of white bumps on the foreskin is Pearly Penile Papules. These are small, harmless bumps that typically appear around the corona of the glans (the head of the penis) and are often mistaken for sexually transmitted infections. Pearly penile papules are a normal anatomical variant and do not require treatment unless they cause psychological distress.

Another possibility is Fordyce spots, which are small, painless, pale bumps that can appear on the shaft of the penis or the scrotum. They are sebaceous glands that are not associated with hair follicles and are completely benign.

If you are noticing a white substance that can be expressed from these bumps, it could also be smegma, which is a combination of dead skin cells, oils, and other bodily secretions that can accumulate under the foreskin if it is not regularly cleaned. Smegma can appear as a white, cheesy substance and is more common in uncircumcised males. Regular hygiene practices can help prevent its buildup.

While herpes typically presents with painful blisters or sores, the absence of pain or itching in your case makes it less likely that this is the cause. However, if you have any doubts or if the bumps change in appearance, become painful, or are accompanied by other symptoms, it is important to consult a healthcare professional for a proper diagnosis.

In summary, the white bumps on your foreskin are most likely benign conditions such as pearly penile papules or Fordyce spots, especially given their painless nature and the fact that they recur in the same location. Maintaining good hygiene and monitoring the bumps for any changes is advisable. If you have concerns or if the situation changes, seeking medical advice from a urologist or dermatologist would be the best course of action to ensure peace of mind and proper care.

Based on the literal description, it is not possible to confirm the condition without actually seeing the lesion. It is recommended to have a physician evaluate it to determine the next steps. If necessary, surgery or a biopsy may be required.
