Burning sensation during urination before and after ejaculation?

Hello doctor, I have had this issue since middle school, so I would like to ask about it.
Whether I urinate first and then ejaculate, or ejaculate first and then urinate, I experience a burning sensation.
This situation does not occur frequently; if I ejaculate once a day, it happens about once a month.
The burning sensation in the urethra goes away after a while, and sometimes it disappears after urinating again.
What could this problem be?

Jason, 20~29 year old female. Ask Date: 2020/11/03

Dr. Cai Zhuorong reply Urology

The first step is to perform a urinalysis, and additionally, consider conducting a semen analysis.

More Info

The burning sensation during urination, especially before and after ejaculation, can be a concerning symptom and may indicate several underlying issues. Given your description, it appears that this sensation is not a constant issue but occurs intermittently, roughly once a month, and is relieved sometimes by urination.

Possible Causes
1. Urinary Tract Infection (UTI): One of the most common causes of burning during urination is a urinary tract infection. UTIs can cause inflammation and irritation in the urinary tract, leading to discomfort during urination. Although you mentioned that this occurs infrequently, it is still a possibility, especially if you have had recurrent UTIs in the past.

2. Urethritis: This is an inflammation of the urethra, which can be caused by infections (bacterial or viral) or irritants. If you have had any recent sexual activity, sexually transmitted infections (STIs) such as chlamydia or gonorrhea could also be a cause.

3. Prostatitis: In males, inflammation of the prostate gland can lead to symptoms such as burning during urination and ejaculation. This condition can be acute or chronic and may require medical evaluation.

4. Irritation from Products: Sometimes, soaps, lotions, or even certain laundry detergents can irritate the genital area, leading to a burning sensation. If you have recently changed any products you use, this could be a contributing factor.

5. Dehydration or Concentrated Urine: If you are not drinking enough fluids, your urine may become concentrated, which can irritate the bladder and urethra, leading to a burning sensation.

6. Nerve Sensitivity: Some individuals may have heightened sensitivity in the nerves of the urethra or surrounding areas, which can lead to discomfort during urination or ejaculation.

Solutions and Recommendations
1. Hydration: Ensure you are drinking plenty of water throughout the day. Staying well-hydrated can help dilute your urine and reduce irritation.

2. Avoid Irritants: Pay attention to any products you use in the genital area. Avoid scented soaps, lotions, or other products that may cause irritation.

3. Seek Medical Evaluation: Given that this symptom has persisted since middle school, it would be wise to consult a healthcare provider. They may recommend a urinalysis to check for infections or other abnormalities. If necessary, they might also perform tests for STIs or other conditions.

4. Monitor Symptoms: Keep track of when the burning sensation occurs, its duration, and any other accompanying symptoms. This information can be helpful for your healthcare provider in diagnosing the issue.

5. Avoid Sexual Activity During Symptoms: If you experience burning during urination, it may be best to avoid sexual activity until you have consulted with a healthcare provider to prevent potential irritation or further complications.

6. Consider a Urologist: If your primary care physician cannot identify the issue, a referral to a urologist may be beneficial. They specialize in urinary tract and male reproductive health and can provide more targeted evaluations and treatments.

In conclusion, while occasional burning during urination may not always indicate a serious problem, it is essential to address persistent or recurrent symptoms with a healthcare professional. They can provide a thorough evaluation and appropriate treatment options tailored to your specific situation.
