Burning pain sensation
Hello Dr.
Hsu: After ejaculation, I often experience a painful burning sensation when urinating (sometimes it occurs, sometimes it doesn't), and I always feel like I haven't completely emptied my bladder (as if something is stuck inside).
It usually resolves after a day.
Recently, after ejaculation, I had a very painful burning sensation in the penis when I urinated.
What could be causing this? Thank you.

The burning sensation and pain during urination after ejaculation that you are experiencing can be attributed to several potential causes. It's important to understand that while occasional discomfort may not be alarming, recurrent symptoms warrant a thorough evaluation by a healthcare professional.
One common cause of post-ejaculation pain and burning sensation is prostatitis, which is inflammation of the prostate gland. Prostatitis can be acute or chronic and may result from bacterial infection or other non-bacterial factors. Symptoms often include pain in the pelvic area, discomfort during urination, and sometimes flu-like symptoms if the condition is acute. Chronic prostatitis may not always present with clear signs of infection, making it more challenging to diagnose.
Another possibility is urethritis, which is inflammation of the urethra, often caused by sexually transmitted infections (STIs) such as gonorrhea or chlamydia. Urethritis can lead to burning sensations during urination, and it may be accompanied by discharge from the penis. If you have had unprotected sexual encounters, it is crucial to consider this possibility and get tested.
Epididymitis, an inflammation of the epididymis (the tube that carries sperm), can also cause discomfort after ejaculation. This condition can be caused by infections or trauma and may present with swelling and pain in the scrotum, along with urinary symptoms.
In some cases, pelvic floor dysfunction or muscle tension in the pelvic area can lead to pain during or after ejaculation. Stress and anxiety can exacerbate these symptoms, leading to a cycle of discomfort.
Additionally, if you are experiencing a sensation of incomplete emptying after urination, it could indicate bladder dysfunction or an issue with the prostate. This feeling can be frustrating and may require further investigation to rule out any underlying conditions.
Given the variability of your symptoms—sometimes present and sometimes absent—it is essential to consult a healthcare provider for a comprehensive evaluation. They may perform a physical examination, take a detailed medical history, and possibly order tests such as urinalysis, STI screening, or imaging studies to determine the underlying cause of your symptoms.
In the meantime, here are some general recommendations to help manage your symptoms:
1. Stay Hydrated: Drinking plenty of water can help dilute your urine and may reduce irritation during urination.
2. Avoid Irritants: Limit or avoid caffeine, alcohol, and spicy foods, as these can irritate the bladder and exacerbate symptoms.
3. Practice Good Hygiene: Ensure proper genital hygiene to reduce the risk of infections.
4. Manage Stress: Since stress can contribute to pelvic tension, consider relaxation techniques such as deep breathing, meditation, or yoga.
5. Seek Medical Attention: If symptoms persist or worsen, do not hesitate to seek medical advice. Early intervention can prevent complications and lead to more effective treatment.
In conclusion, while occasional burning sensations after ejaculation may not be a cause for concern, recurrent symptoms should be evaluated by a healthcare professional. Conditions such as prostatitis, urethritis, or pelvic floor dysfunction could be responsible, and appropriate diagnosis and treatment are essential for your health and well-being.
