Dry and painful sensation in the nose and philtrum area?

Hello, recently the weather has turned cooler, and my nose, as usual during cold weather, has become dry and painful.
The area inside my nostrils down to the philtrum hurts when touched, and it cracks and forms scabs that are difficult to heal.
Sometimes it develops yellowish, pus-like substances.
What can I do to prevent this issue? Would wearing a mask or applying lotion help? I already drink plenty of water.
Thank you for your assistance.

Zhan, 20~29 year old female. Ask Date: 2020/11/08

Dr. Ye Qianyu reply Family Medicine

Hello! Based on your description, it is recommended to apply petroleum jelly (lip balm can also be used) on the cracked area.
Wearing a mask can help prevent cold air and dirt from coming into contact, but if the mask becomes wet with saliva, it is advisable to change it to avoid infection.

Dealing with dryness and pain in the nose and philtrum, especially during colder weather, can be quite uncomfortable. The symptoms you describe, such as pain, cracking, and crusting in the nasal passages and philtrum (the area between the nose and upper lip), are common during dry or cold seasons. Here are some strategies to alleviate these symptoms and promote healing.

1. Hydration:
While you mentioned that you are already drinking a lot of water, it's essential to maintain adequate hydration, especially in dry environments. Dehydration can exacerbate dryness in mucous membranes. Consider using a humidifier in your home, particularly in your bedroom while you sleep. This can help maintain moisture in the air, reducing dryness in your nasal passages.

2. Nasal Saline Sprays:
Using saline nasal sprays can be very effective in keeping your nasal passages moist. These sprays help to hydrate the mucous membranes and can also aid in clearing out any crusting or debris. You can use them several times a day, especially before bed or when you feel dryness.

3. Topical Treatments:
Applying a thin layer of a gentle, fragrance-free moisturizer or ointment (like petroleum jelly or a specialized nasal ointment) to the inside of your nostrils can help prevent cracking and soothe irritation. Be cautious not to apply too much, as this can lead to a buildup of product.

4. Avoid Irritants:
Try to avoid irritants such as smoke, strong odors, and allergens that can exacerbate nasal dryness and discomfort. If you are in a cold environment, wearing a mask can help trap moisture and protect your nose from cold air.

5. Gentle Cleaning:
If you notice crusting or yellowish discharge, it’s important to clean your nose gently. Use a saline solution or a soft, damp cloth to wipe away any crusts. Avoid picking at the area, as this can lead to further irritation or bleeding.

6. Dietary Considerations:
Incorporate foods rich in omega-3 fatty acids (like fish, flaxseeds, and walnuts) and antioxidants (like fruits and vegetables) into your diet. These can help support skin and mucous membrane health.

7. Consult a Healthcare Provider:
If your symptoms persist or worsen, it’s advisable to consult with a healthcare provider. They can assess for any underlying conditions such as allergies, infections, or skin conditions that may require specific treatments. In some cases, prescription medications or topical treatments may be necessary.

8. Avoid Overuse of Decongestants:
If you are using over-the-counter decongestant nasal sprays, be cautious about overuse, as they can lead to rebound congestion and further dryness.

Conclusion:
In summary, managing dryness and pain in the nose and philtrum involves a combination of hydration, gentle care, and avoidance of irritants. Using saline sprays, moisturizers, and maintaining a humid environment can significantly alleviate your symptoms. If these measures do not provide relief, seeking medical advice is crucial to rule out any underlying issues. Remember, taking care of your nasal health is essential, especially during the colder months.
