Chest pain in specific positions?
Hello, doctor.
I have had trouble sleeping easily since my student days.
In the past three to four years, I have noticed that sleeping in a position where my body is flat, my arms are raised, and my head is resting on the pillow helps me fall asleep better; so I have mostly maintained this sleeping posture.
However, in the past few weeks, when I sleep in the same position, I experience a "pulling sensation to the side" below my sternum, it feels hollow in the center when touched, and I also have some chest pain (which I did not experience before).
(If I switch to sleeping on my side or lie flat with my arms down, I do not have this sensation; but changing my sleeping position makes it difficult to fall asleep.) I would like to know what might be causing this.
I have had mild arrhythmia and mild gastroesophageal reflux for a long time, and I recently lost 5 kilograms (but my BMI is still classified as overweight); could these symptoms be related? I really want to sleep well, and I thank you in advance for your response.

Dear Ms.
H:
1.
When a clinician addresses the issue of chest pain, the first four critical conditions to rule out are myocardial infarction, pulmonary embolism, pneumothorax, and aortic dissection.
If none of these conditions are present, differential diagnoses should be considered based on the chief complaint and symptoms, including lung tumors, lung abscesses, pneumonia, tuberculosis, arrhythmias, congenital heart disease, myocardial ischemia, pericardial issues, gastroesophageal reflux disease, esophageal tumors, esophageal structural abnormalities, musculoskeletal problems, and neuritis, among others.
Additionally, anxiety disorders, hypochondriasis, and somatoform disorders should also be taken into account.
2.
You have a history of mild arrhythmia and mild gastroesophageal reflux, and you have recently lost 5 kilograms; however, your body mass index (BMI) is still elevated.
Therefore, it is not possible to completely rule out a direct relationship between your chest pain and these factors; of course, the human body functions as a whole, and an indirect relationship is likely present.
3.
It is advisable for you to discuss and undergo a thorough examination with your family physician or attending physician; it is not appropriate to speculate without proper evaluation.

More Info
Chest pain when lying down can be concerning, especially when it disrupts your sleep. Based on your description, there are several potential causes for the discomfort you are experiencing, and understanding these can help guide you toward appropriate solutions.
Possible Causes of Chest Pain When Lying Down
1. Gastroesophageal Reflux Disease (GERD): Given your history of mild gastroesophageal reflux, it's possible that lying flat exacerbates acid reflux symptoms. When you lie down, stomach acid can more easily flow back into the esophagus, causing a burning sensation or pain in the chest area. This is often accompanied by a feeling of pressure or discomfort in the chest, especially if you have not eaten for a while or have consumed certain foods that trigger reflux.
2. Musculoskeletal Issues: The "pulling" sensation you describe could also be related to musculoskeletal problems. Poor posture while sleeping or tension in the chest muscles can lead to discomfort. If your sleeping position has changed over the years, it might be putting additional strain on your chest muscles or ribs, leading to pain when lying down.
3. Anxiety and Stress: Anxiety can manifest physically in many ways, including chest pain. If you are experiencing stress or anxiety, it could contribute to muscle tension and discomfort in your chest, particularly when you are trying to relax or sleep.
4. Cardiac Concerns: Although less likely given your description, it is essential to rule out any cardiac issues, especially since you mentioned having mild arrhythmia. Chest pain can sometimes be related to heart problems, and it’s crucial to consult a healthcare provider to ensure that your heart is healthy.
5. Weight Changes: You mentioned losing 5 kg but still being classified as overweight. Weight changes can affect your body in various ways, including how your body handles stress and pressure on the chest. Excess weight can contribute to GERD and other conditions that may cause discomfort.
Recommendations for Management
1. Adjust Sleeping Position: Since changing your sleeping position seems to alleviate the pain, consider using pillows to elevate your upper body while sleeping. This can help reduce the likelihood of acid reflux and may relieve pressure on your chest.
2. Dietary Modifications: Pay attention to your diet, especially in the hours leading up to bedtime. Avoiding heavy meals, caffeine, and spicy foods can help reduce reflux symptoms. Eating smaller meals throughout the day may also be beneficial.
3. Stress Management: Incorporating relaxation techniques such as deep breathing, meditation, or gentle yoga before bed can help reduce anxiety and muscle tension, potentially alleviating chest discomfort.
4. Consult a Healthcare Provider: Given your symptoms and medical history, it is advisable to consult a healthcare provider. They may recommend tests such as an EKG, echocardiogram, or upper GI series to rule out any serious conditions. Discussing your symptoms with a cardiologist or gastroenterologist may provide further insights.
5. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their intensity, and any associated activities or foods. This information can be helpful for your healthcare provider in determining the underlying cause.
6. Medication Review: If you are taking medications for your heart rhythm or any other condition, review these with your doctor to ensure they are not contributing to your symptoms.
Conclusion
Chest pain when lying down can stem from various causes, including GERD, musculoskeletal issues, anxiety, and potential cardiac concerns. It is essential to address these symptoms proactively, especially since they affect your sleep. By making lifestyle adjustments, consulting with healthcare professionals, and monitoring your symptoms, you can work toward finding relief and improving your overall well-being. Remember, your health is paramount, and seeking help is a crucial step in managing your symptoms effectively.
