Frequent urination and kidney stones?

My boyfriend, who is 22 years old, recently discovered an increase in urination frequency before he enlisted in the military.
After seeing a doctor, he was diagnosed with urethritis.
Although the inflammation has resolved after taking medication, he still experiences frequent urination.
Later, he visited a larger hospital and found out he had kidney stones in his right kidney.
He was prescribed medication and advised to drink more water.
After undergoing an MRI, it was confirmed that the stones were gone and the hydronephrosis issue had also resolved, but the frequent urination has not improved.
I would like to ask the doctor why this condition persists.

Dr. Li Xinyi reply Urology

Based on your description, it is indeed a fact that you have urethritis.
As for the stones, they do not necessarily seem to be related.
It is recommended that you visit a urologist or an infectious disease specialist to thoroughly examine and treat the urethritis.

Frequent urination can be a perplexing symptom, especially following a diagnosis of kidney stones and urinary tract inflammation. In your boyfriend's case, it appears that while the initial causes of his symptoms—namely the urinary tract infection (UTI) and kidney stones—have been addressed, the issue of frequent urination persists. This can be attributed to several factors, which I will elaborate on below.

Firstly, it’s important to understand that urinary frequency can be influenced by various factors, including bladder sensitivity, inflammation, and even psychological factors. After an episode of urinary tract infection, the bladder may become more sensitive or irritable, leading to a condition known as "overactive bladder." This condition can cause a person to feel the need to urinate more frequently, even when the bladder is not full. The inflammation from the previous infection can take time to resolve completely, and during this period, the bladder may react by signaling the need to void more often.

Secondly, even after the resolution of kidney stones, residual effects can linger. The urinary tract may still be recovering from the irritation caused by the stones and any associated inflammation. This irritation can lead to increased urgency and frequency of urination. Additionally, if there was any damage to the urinary tract during the passage of stones or due to the inflammation, it may take time for the tissues to heal fully, which can also contribute to ongoing symptoms.

Moreover, psychological factors such as anxiety can exacerbate urinary frequency. If your boyfriend is concerned about his health or has had a stressful experience related to his kidney stones or urinary issues, this could lead to increased awareness of his bladder and a heightened perception of the need to urinate.

In terms of management, it is essential for your boyfriend to continue following up with his healthcare provider. If the frequent urination persists, further evaluation may be warranted. This could include urodynamic studies to assess bladder function or a referral to a urologist for specialized care. Additionally, lifestyle modifications such as bladder training exercises, pelvic floor exercises, and dietary adjustments may help manage symptoms.

Hydration is crucial, especially after kidney stone issues, but it’s also important to balance fluid intake. Excessive fluid intake can sometimes lead to more frequent urination, so finding a comfortable level of hydration is key. It may also be beneficial to avoid bladder irritants such as caffeine, alcohol, and spicy foods, which can exacerbate urinary frequency.

In conclusion, while the resolution of the urinary tract infection and kidney stones is a positive development, the persistence of frequent urination can be attributed to bladder sensitivity, residual inflammation, and possibly psychological factors. Continued monitoring and a comprehensive approach to treatment will be essential in addressing this ongoing symptom. Encourage your boyfriend to maintain open communication with his healthcare provider to explore further options for relief and management.
