Medication Consultation for Buprenorphine
Hello Dr.
Wang: I have been taking Prozac for over three weeks (for obsessive-compulsive disorder).
I am currently a student retaking exams and taking one and a half pills (the doctor said to increase to two pills, but I feel it's not okay right now).
I have been dreaming every night and wake up feeling very tired, yawning a lot, and my eyes feel heavy.
I'm not sure if this is due to poor sleep or excessive daytime sleepiness.
I have also noticed that when I take it in the morning, my reaction time seems to slow down (which is not ideal for studying).
I’m unsure when to take the medication or what would be the best course of action.
Thank you very much for your response.

Hello: The effects of antidepressants on mood improvement typically become more noticeable after 2-3 weeks, but side effects often appear earlier than the benefits.
However, these side effects generally diminish with continued use of the medication.
If one does not understand this phenomenon and feels "unwell after taking the medication," it can lead to a lack of trust in the treatment and sometimes result in abrupt discontinuation of the medication.
Some side effects can be monitored temporarily before deciding how to manage them, while others require immediate attention.
If you experience fatigue and vivid dreams after taking Wellbutrin in the morning or at bedtime, it is advisable to change the dosing to after dinner.
If there is no improvement, please discuss this with your current physician and avoid discontinuing the medication on your own, as it may negate the progress made.
I hope this answers your question :)

Managing medication for obsessive-compulsive disorder (OCD) can be challenging, especially when considering the side effects and their impact on daily functioning, such as studying for exams. In your case, you are currently taking Lexapro (escitalopram), which is a selective serotonin reuptake inhibitor (SSRI) commonly prescribed for OCD. You mentioned that you have been on this medication for over three weeks and are currently taking 1.5 pills, with a recommendation from your doctor to increase to 2 pills.
Firstly, it’s important to understand that SSRIs like Lexapro can take several weeks to reach their full therapeutic effect. While some individuals may experience improvements in their symptoms sooner, others may need to wait longer. Side effects, such as fatigue, drowsiness, and vivid dreams, are not uncommon when starting or adjusting the dosage of SSRIs. These side effects can be particularly concerning for students who need to maintain focus and energy levels for studying.
You mentioned feeling tired upon waking and experiencing excessive yawning, which could be attributed to the medication. It’s also possible that your sleep disturbances, such as vivid dreams, could be affecting your overall sleep quality, leading to daytime fatigue. It’s crucial to assess whether these symptoms are due to the medication or other factors, such as stress from studying or changes in your sleep environment.
Here are some strategies to consider:
1. Timing of Medication: If you are experiencing drowsiness during the day, you might want to discuss with your doctor the possibility of taking your medication at a different time. Some individuals find that taking their SSRI in the morning helps reduce daytime drowsiness, while others may prefer taking it at night to minimize side effects like insomnia.
2. Gradual Dose Adjustment: If you are hesitant about increasing your dose to 2 pills, it’s essential to communicate this with your healthcare provider. They can help you weigh the benefits of a higher dose against the side effects you are experiencing. Sometimes, a slower titration to the desired dose can help mitigate side effects.
3. Sleep Hygiene: Improving your sleep hygiene can also help alleviate some of the fatigue you are experiencing. This includes establishing a regular sleep schedule, creating a comfortable sleep environment, and avoiding stimulants like caffeine close to bedtime.
4. Monitoring Side Effects: Keep a journal of your symptoms, including your mood, energy levels, and any side effects you experience. This information can be valuable for your healthcare provider in determining the best course of action.
5. Therapeutic Support: In addition to medication, cognitive-behavioral therapy (CBT) is an effective treatment for OCD. If you are not already engaged in therapy, consider discussing this option with your doctor. Therapy can provide you with coping strategies to manage your symptoms and reduce anxiety.
6. Consulting Your Doctor: It’s crucial to maintain open communication with your healthcare provider about your concerns. They can provide guidance tailored to your specific situation and help you navigate the complexities of managing your medication while preparing for exams.
In summary, while SSRIs like Lexapro can be effective in managing OCD symptoms, they can also come with side effects that may impact your daily life, especially as a student. By discussing your concerns with your healthcare provider, adjusting the timing of your medication, and implementing strategies to improve your sleep and overall well-being, you can work towards finding a balance that allows you to manage your OCD effectively while performing well in your studies.
