Occasional discomfort after inguinal hernia surgery?
Hello Doctor: I have a hernia where my intestine descended into the right testicle.
I discovered it seven years ago and underwent surgery this February.
Since then, I have been very careful with my recovery; I only started lifting heavier objects three months post-surgery and began exercising six months later.
However, as of now, at the end of November, I still frequently feel discomfort at the surgical site.
During my follow-up appointment in February, the doctor mentioned that I might experience a tight sensation, but I feel it is more uncomfortable.
This discomfort usually occurs when I elevate my right leg, experience urgency to urinate, need to have a bowel movement (and if I don't go, the discomfort persists), or when I lean back in a chair.
What concerns me the most is that my right testicle has become very sensitive.
If I move too vigorously while walking and my right testicle hits my thigh, it causes significant discomfort that radiates from the testicle to the surgical site.
Due to my overweight condition, I am hesitant to exercise for weight loss.
I can manage light jogging, but any jumping, even small jumps, causes discomfort in my right testicle.
Is this normal?

Because there was a considerable time interval between the discovery of your hernia and the actual surgery, based on your description of the hernia descending into the testicle (which should refer to the scrotum), it indicates that your hernia sac likely extended into the scrotal area at that time.
A larger hernia repair may result in prolonged abnormal sensations, possibly due to scar tissue effects from the extended surgical area causing localized nerve compression and discomfort.
Some individuals may gradually adapt over time (for example, experiencing more severe discomfort initially, which improves as time goes on).
However, for others, the discomfort may persist or even worsen.
If necessary, you can consider taking oral acetaminophen to alleviate the discomfort, such as taking a dose before exercising to see if the discomfort becomes more manageable.
It is still advisable to discuss this further with your original surgeon if needed.

Post-surgery discomfort after inguinal hernia repair is a common concern among patients, and your experience reflects some typical symptoms that can arise during the recovery process. After undergoing surgery for an inguinal hernia, it is not unusual to experience discomfort or sensitivity in the surgical area, especially if the surgery involved the manipulation of tissues and nerves in that region.
Firstly, it is important to understand that the recovery from inguinal hernia repair can vary significantly from person to person. While many patients may start to feel better within a few weeks, others may experience lingering discomfort for several months. The sensation of tightness or discomfort you mentioned, particularly when engaging in certain movements or activities, is often due to the healing process of the tissues and the nerves that may have been affected during surgery.
The sensitivity of your right testicle, particularly when it comes into contact with your thigh or during certain movements, can also be attributed to nerve irritation or inflammation in the area. This is not uncommon, especially if the surgical procedure involved manipulation near the spermatic cord or surrounding structures. The nerves in this region can take time to heal, and during this period, you may experience heightened sensitivity or discomfort.
Regarding your concerns about specific activities, such as lifting heavy objects or engaging in high-impact exercises, it is wise to proceed with caution. While it is commendable that you have been careful with your recovery, it is essential to listen to your body. If certain movements consistently cause discomfort, it may be best to avoid them until you have fully healed. Gradually increasing your activity level, as you have done, is a good approach, but be mindful of any pain signals your body is sending.
If you are experiencing persistent discomfort, especially if it is affecting your quality of life or your ability to engage in physical activities, it would be prudent to follow up with your surgeon or a healthcare provider. They can assess your situation more thoroughly, possibly through a physical examination or imaging studies, to ensure that there are no complications such as fluid accumulation, infection, or recurrence of the hernia.
In summary, while some discomfort after inguinal hernia repair can be normal, especially in the early stages of recovery, persistent or worsening symptoms should not be ignored. It is crucial to maintain open communication with your healthcare provider to address any concerns and ensure a smooth recovery process. Remember, every individual's healing journey is unique, and your comfort and well-being should always be a priority.
