Post-Circumcision: Severe Swelling One Week After Surgery - Urology

One week after circumcision, there is severe swelling after the dressing is removed?

Hello Doctor: I underwent a circumcision procedure using laser one week ago.
I have been keeping it covered post-operatively, and during dressing changes, there has been no significant swelling observed.
However, on the sixth day post-surgery, when I removed the bandage, the swelling became very severe.
Today (exactly one week post-surgery), I called to consult with the doctor, but he only mentioned that the swelling is a normal phenomenon.
Since I need to travel in three weeks, which will require frequent movement and insufficient rest, I would like to ask if this is within the range of normal conditions? Do I need to undergo any additional treatment? Or is there any other treatment available to reduce the swelling quickly?

Jinghong, 20~29 year old female. Ask Date: 2020/11/30

Dr. Du Shixi reply Urology

Hello: Postoperative swelling and bruising are normal and will gradually subside.
Please rest more, the wound looks okay, just keep it clean.
Wishing you good health.

More Info

After undergoing a circumcision procedure, it is not uncommon to experience some degree of swelling in the surgical area. However, severe swelling one week post-surgery can raise concerns and warrants a closer examination. Here’s a comprehensive understanding of your situation based on the information provided.

Normal Post-Operative Swelling
1. Expected Swelling: After any surgical procedure, including circumcision, some swelling is expected. This is part of the body's natural healing response. The swelling may peak around the first week post-surgery and then gradually decrease as healing progresses.

2. Signs of Normal Healing: If the swelling is accompanied by mild discomfort, redness, or tenderness, it may still fall within the normal range. However, if the swelling is severe, persistent, or accompanied by other symptoms such as fever, increased pain, or discharge, it may indicate a complication.

Possible Causes of Severe Swelling
1. Fluid Accumulation: The body may retain fluid in the surgical area, leading to significant swelling. This can occur due to inflammation or the body's response to the surgical trauma.

2. Infection: While you mentioned that there was no severe swelling during the initial days post-surgery, an infection can develop later. Signs of infection include increased redness, warmth, pus or discharge, and systemic symptoms like fever.

3. Allergic Reaction: Sometimes, swelling can be a reaction to medications, topical ointments, or even the sutures used during the procedure.

Management and Treatment Options
1. Rest and Elevation: If possible, try to keep the area elevated to reduce swelling. Avoid strenuous activities that may exacerbate the swelling.

2. Cold Compresses: Applying a cold compress to the area can help reduce swelling and provide relief. Ensure that the compress is not applied directly to the skin; use a cloth barrier.

3. Medication: Over-the-counter anti-inflammatory medications, such as ibuprofen, can help reduce swelling and discomfort. However, consult your physician before taking any medication, especially if you have other health conditions or are on other medications.

4. Follow-Up with Your Surgeon: Since you have a trip planned in three weeks, it is crucial to communicate your concerns with your surgeon. They may want to examine the swelling to rule out any complications and provide tailored advice based on your healing progress.

5. Avoiding Irritants: Ensure that the area is kept clean and dry. Avoid any products that may irritate the skin, including certain soaps or lotions.

When to Seek Immediate Care
If you experience any of the following, seek medical attention promptly:
- Increased swelling that does not improve with rest or cold compresses.

- Signs of infection (redness, warmth, pus, fever).

- Severe pain that is not relieved by over-the-counter medications.

- Any unusual changes in the surgical site.

Conclusion
In summary, while some swelling is expected after circumcision, severe swelling one week post-surgery should be monitored closely. It is essential to maintain communication with your healthcare provider, especially given your upcoming travel plans. They can provide the best guidance on managing your symptoms and ensuring a smooth recovery. If the swelling persists or worsens, do not hesitate to seek further evaluation to rule out any complications.
