Postoperative care after circumcision?
Hello doctor, I underwent circumcision surgery, and on the same day, my penile shaft became engorged and swollen (about three times its normal size).
Additionally, there is a soft, movable lump that appears, which causes slight discomfort when pressed.
As of today, one week post-surgery, there has been no significant reduction in size.
I would like to know what this is and if it will go away.
Thank you.

Hello: For swelling, it is advisable to consult a physician, as it is often a hematoma.
Wishing you good health.

After a circumcision, it is not uncommon to experience some swelling and discomfort in the area. The swelling you are describing, where the shaft of the penis appears enlarged and there is a soft, movable lump, can be attributed to several factors related to the surgical procedure.
Firstly, it is important to understand that swelling is a natural response of the body to surgery. When tissue is cut, the body sends extra blood and fluid to the area to promote healing, which can lead to temporary swelling. This is often referred to as "post-operative edema." In most cases, this swelling should gradually decrease over time as the healing process continues.
The lump you are feeling could be a hematoma, which is a localized collection of blood outside of blood vessels, often resulting from bleeding during or after the surgery. Hematomas can feel soft and movable, and while they may cause some discomfort, they typically resolve on their own as the body reabsorbs the blood.
In your case, since you mentioned that the swelling has not significantly decreased after one week, it is advisable to monitor the situation closely. While some swelling can persist for a week or two, if you notice any of the following symptoms, you should seek medical attention:
1. Increased Pain: If the pain becomes more severe rather than improving, it could indicate a complication.
2. Signs of Infection: Redness, warmth, or discharge from the surgical site could suggest an infection.
3. Persistent Swelling: If the swelling does not begin to decrease after two weeks, it may warrant further evaluation.
4. Fever: A fever could indicate an infection or other complications.
In terms of management, here are some general recommendations:
- Rest: Avoid strenuous activities that could exacerbate swelling or discomfort.
- Ice Application: Applying ice packs to the area for 15-20 minutes at a time can help reduce swelling and alleviate pain. Make sure to wrap the ice pack in a cloth to avoid direct contact with the skin.
- Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage discomfort. However, consult your doctor before taking any medication.
- Follow-Up Care: It is essential to attend any scheduled follow-up appointments with your surgeon. They can assess the healing process and address any concerns you may have.
In conclusion, while some swelling and discomfort are normal after circumcision, persistent or worsening symptoms should not be ignored. If you have any concerns about your recovery or if the swelling does not improve, it is best to consult your healthcare provider for a thorough evaluation. They can provide personalized advice and determine if any further intervention is necessary. Remember, your health and comfort are paramount, and seeking help when needed is always the right choice.
