Post-Circumcision Complications: Swelling and Pain Concerns - Urology

Is it normal?

Hello doctor, I underwent a circumcision three months ago.
The surgery went smoothly, but during a dressing change by a different physician, the dressing technique was slightly different, and it may have been too tight.
This led to significant swelling on the lower left side of the glans.
It was almost two weeks before I started soaking in baths.
Now, nearly four months later, I have noticed persistent swelling on the lower left side of my penis, and there is a hard lump that is painful to touch.
Sometimes, I also experience pain during erections.
What could be the cause of this? Additionally, the area around the suture feels hard and irregular, but there are no such issues on the shaft of the penis.
Occasionally, I also feel pain when touching that area.
After the stitches were removed, I felt a prickling sensation, but I was told that the stitches would be absorbed, so I didn't pay much attention to it.
What should I be concerned about? Also, when will the color difference in the skin after the circumcision change? Thank you.

Hello, after circumcision surgery, there may be localized swelling due to the cutting of lymphatic vessels and blood vessels.
This is unrelated to the dressing change method.
Tightening the dressing after changing it can help prevent swelling of the wound.
Generally, we assess the condition of the wound and apply slight pressure.
Over time, it will gradually heal.
You can apply local heat to accelerate tissue repair, which will also help reduce swelling more quickly.
If there are any lumps, local heat application or gentle massage may also be beneficial.

Post-circumcision complications, such as swelling and pain, can be concerning and may arise from various factors. After a circumcision, it is not uncommon for patients to experience some degree of swelling and discomfort as part of the healing process. However, persistent swelling, pain, or the presence of hard lumps can indicate complications that should be addressed.

In your case, it appears that you experienced significant swelling in the left lower part of the glans after a dressing change, which may have been due to the dressing being too tight. This can lead to localized edema and discomfort. It is essential to ensure that dressings are applied correctly to avoid constriction, which can impede blood flow and cause swelling.

The hard lump you mentioned could be a few things. One possibility is that it could be scar tissue forming as part of the healing process. Scar tissue can sometimes feel firm or hard and may be tender to the touch. Another possibility is that you may have developed a condition known as a hematoma, which is a localized collection of blood outside of blood vessels, often resulting from trauma or surgical procedures. Hematomas can cause swelling and pain, particularly if they become large or infected.

Regarding the pain during erections, this could be due to several factors, including inflammation, scar tissue, or even nerve irritation. If the pain persists or worsens, it is crucial to consult with your healthcare provider for a thorough evaluation. They may recommend imaging studies or a physical examination to assess the area and determine the cause of your symptoms.

As for the irregularity and firmness around the suture line, it is common for absorbable sutures to cause some localized irritation or discomfort as they dissolve. However, if you notice any signs of infection (such as increased redness, warmth, or discharge), or if the pain becomes severe, you should seek medical attention promptly.

Regarding the color change of the skin after circumcision, it can take time for the skin to return to a more normal appearance. Initially, the area may appear red or darker due to increased blood flow and healing. Over time, as the swelling decreases and the tissue heals, the color should gradually normalize. This process can vary from person to person, depending on individual healing rates and skin types.

In summary, while some swelling and discomfort are expected after circumcision, persistent symptoms warrant further investigation. It is essential to communicate openly with your healthcare provider about your concerns, as they can provide tailored advice and treatment options based on your specific situation. If you have not done so already, consider scheduling a follow-up appointment to discuss your symptoms in detail and explore potential interventions to alleviate your discomfort.
