Recurrent epididymitis?
Hello Doctor, I had sexual intercourse in August, and about three weeks later, I was diagnosed with epididymitis at the clinic.
I took medication and recovered, but I have had recurrent inflammation since then, and this is now the third time.
I currently have a hard lump in the epididymis.
1.
What are the symptoms of chronic epididymitis? Does recurrent inflammation count as chronic epididymitis, and is it possible to fully recover from it?
2.
I am also experiencing weak urine flow and frequent urination.
Could this indicate that the prostate is also inflamed, or could there be other underlying issues? Thank you.

Dr. Du Shixi reply Urology
Hello: 1) If the lump starts to soften gradually and becomes smaller without any pain, it indicates that the treatment is effective, and it is likely that it will not recur.
However, a small number of patients may experience a recurrence of symptoms and will require further treatment.
2) If there are accompanying urinary symptoms, you should consult a urologist at a hospital.
Acute prostatitis may cause fever, while chronic prostatitis is more difficult to diagnose and requires regular follow-up visits.
It is recommended to get checked at a major hospital.
Wishing you good health.

More Info
Chronic epididymitis is a condition characterized by inflammation of the epididymis, a coiled tube located at the back of the testicle that stores and carries sperm. Understanding its symptoms, causes, and treatment options is crucial for effective management, especially in cases of recurrent inflammation.
Symptoms of Chronic Epididymitis
The symptoms of chronic epididymitis can vary but typically include:
1. Persistent Pain: Unlike acute epididymitis, which presents with sudden and severe pain, chronic epididymitis may cause a dull, aching pain in the scrotum or groin that persists over time.
2. Swelling: The affected epididymis may appear swollen or enlarged, and there may be tenderness upon palpation.
3. Discomfort During Sexual Activity: Patients may experience discomfort or pain during ejaculation or sexual intercourse.
4. Changes in Urination: Symptoms may include increased frequency of urination, urgency, or discomfort during urination.
5. Hard Lump: As you mentioned, the presence of a hard lump in the epididymis can be a sign of chronic inflammation or scarring.
Causes of Chronic Epididymitis
Chronic epididymitis can result from several factors, including:
1. Infections: Recurrent infections, often due to sexually transmitted infections (STIs) such as chlamydia or gonorrhea, can lead to chronic inflammation.
2. Non-infectious Causes: Conditions such as trauma, prolonged sitting, or certain medical procedures can also contribute to chronic epididymitis.
3. Autoimmune Disorders: In some cases, the body’s immune system may mistakenly attack the epididymis, leading to inflammation.
4. Obstruction: Blockages in the reproductive tract can lead to fluid accumulation and subsequent inflammation.
Is Recurrent Inflammation Considered Chronic Epididymitis?
Yes, recurrent episodes of inflammation can be classified as chronic epididymitis, especially if the symptoms persist over a prolonged period or if there is a pattern of flare-ups. Chronic conditions can sometimes be challenging to treat, but with appropriate management, many patients can achieve significant relief.
Can Chronic Epididymitis Be Cured?
While chronic epididymitis can be managed effectively, complete resolution may not always be possible, especially if there are underlying causes that are not addressed. Treatment typically focuses on alleviating symptoms and preventing recurrence. Options may include:
1. Antibiotics: If an infection is identified, a course of antibiotics may be prescribed. In cases of chronic infection, longer courses may be necessary.
2. Pain Management: Nonsteroidal anti-inflammatory drugs (NSAIDs) can help reduce pain and inflammation.
3. Supportive Care: Wearing supportive underwear, applying ice packs, and avoiding activities that exacerbate symptoms can be beneficial.
4. Surgery: In severe cases, surgical intervention may be necessary to remove the affected epididymis or to address any underlying anatomical issues.
Symptoms of Prostate Inflammation
Regarding your symptoms of weak urination and increased frequency, these could indeed suggest prostatitis, which is inflammation of the prostate gland. Prostatitis can occur alongside epididymitis, especially if there is an underlying infection. Symptoms of prostatitis may include:
- Painful urination
- Difficulty starting or stopping urination
- Pain in the lower back, perineum, or bladder area
- Painful ejaculation
Conclusion
In summary, chronic epididymitis can present with persistent pain, swelling, and changes in urination. Recurrent episodes can be classified as chronic, and while complete resolution may not always be achievable, effective management strategies exist. Your accompanying urinary symptoms may indicate prostatitis, and it is essential to discuss these concerns with your healthcare provider for a comprehensive evaluation and tailored treatment plan. Regular follow-ups and open communication with your doctor can help monitor your condition and adjust treatment as necessary.
