Pain during bowel movements?
I have had this symptom for about one or two years.
Previously, I would have blood in my stool approximately once a week, but the amount was not much, and it was bright red.
It would usually stop after wiping once, but each time I had a bowel movement, it was painful.
Recently, I have been using over-the-counter medication, which has reduced the bleeding, but I still experience pain during bowel movements.
What should I do about this?

Understanding painful bowel movements can be quite complex, as there are numerous potential causes and underlying conditions that may contribute to this discomfort. Based on your description, it seems you have been experiencing painful bowel movements for one to two years, with intermittent rectal bleeding that has recently decreased due to over-the-counter medication. However, the pain during bowel movements persists, which can be indicative of several gastrointestinal issues.
One of the most common causes of painful bowel movements is anal fissures. These are small tears in the lining of the anus that can occur due to passing hard stools or straining during bowel movements. The pain associated with anal fissures can be sharp and is often accompanied by bright red blood on the toilet paper or in the stool. If you have noticed that the blood is bright red and occurs during or after bowel movements, this could be a sign of an anal fissure.
Another possibility is hemorrhoids, which are swollen blood vessels in the rectal area that can cause pain, itching, and bleeding during bowel movements. Hemorrhoids can be internal or external, and they can become inflamed or thrombosed, leading to increased discomfort. The fact that you have experienced bleeding and pain suggests that this could also be a contributing factor.
Additionally, conditions such as inflammatory bowel disease (IBD), which includes Crohn's disease and ulcerative colitis, can lead to painful bowel movements and rectal bleeding. These conditions are characterized by chronic inflammation of the gastrointestinal tract and may present with symptoms such as abdominal pain, diarrhea, and weight loss. If you have any other symptoms such as significant weight loss, persistent diarrhea, or abdominal cramping, it would be essential to consult a healthcare professional for further evaluation.
Given that you have been managing your symptoms with over-the-counter medications, it is crucial to assess whether these treatments are adequately addressing the underlying issue. While topical treatments may provide temporary relief, they may not resolve the root cause of your pain. It is advisable to consult a healthcare provider who can perform a thorough examination, which may include a digital rectal exam or other diagnostic tests, to determine the exact cause of your symptoms.
In the meantime, there are several strategies you can implement to alleviate discomfort during bowel movements. Increasing dietary fiber intake can help soften stools and reduce straining. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Staying well-hydrated is also essential, as adequate fluid intake can help prevent constipation. If you are experiencing significant pain, you may consider using a stool softener or a mild laxative under the guidance of a healthcare professional.
Additionally, practicing good bowel habits, such as not delaying the urge to have a bowel movement and avoiding excessive straining, can be beneficial. Warm sitz baths may also provide relief by soothing the anal area and promoting relaxation.
In conclusion, while there are several potential causes for your painful bowel movements, including anal fissures, hemorrhoids, and possibly more serious conditions like IBD, it is essential to seek medical advice for a proper diagnosis and treatment plan. Early intervention can help prevent complications and improve your quality of life.
