Ejaculation issues
Hello doctor, previously my sexual experiences with my girlfriend were quite normal, but recently in the last two encounters, I don't know why, but shortly after penetration, I immediately felt the urge to ejaculate and did so, with the duration being much shorter than before.
Additionally, after ejaculation, my penis remained erect and did not deflate, and when I tried again, I ejaculated quickly once more.
Could you please tell me what kind of issue this might be? My erectile function seems normal; I only feel there is a problem with ejaculation.
Furthermore, recently my penis has been experiencing intermittent itching, and I'm not sure if this is related.
On average, I masturbate once every two days and do not have an exercise routine.

More Info
Premature ejaculation (PE) is a common sexual dysfunction that affects many men at some point in their lives. It is characterized by the inability to control ejaculation, leading to ejaculation occurring sooner than desired during sexual activity. This can result in distress for both partners and can affect the overall sexual experience. Understanding the causes and potential solutions for PE is crucial for addressing this issue effectively.
Causes of Premature Ejaculation
1. Psychological Factors: Anxiety, stress, and relationship issues can significantly impact sexual performance. If a man is worried about his sexual abilities or is under pressure to perform, it can lead to premature ejaculation. In your case, the recent changes in your sexual performance could be linked to psychological factors, especially if you are feeling anxious about the situation.
2. Biological Factors: Hormonal imbalances, neurotransmitter levels, and certain medical conditions can contribute to PE. For instance, conditions like hyperthyroidism or prostatitis may affect ejaculation timing. The itching sensation you mentioned could be related to a skin condition or an infection, which might also influence your sexual function.
3. Lifestyle Factors: Lack of physical activity, excessive alcohol consumption, and smoking can negatively impact sexual health. Since you mentioned not having a regular exercise routine, incorporating physical activity could improve your overall health and potentially help with sexual function.
Solutions for Premature Ejaculation
1. Behavioral Techniques: Techniques such as the "stop-start" method or the "squeeze" technique can help you gain better control over ejaculation. The stop-start method involves pausing sexual stimulation before ejaculation, while the squeeze technique involves squeezing the base of the penis to reduce arousal.
2. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through Kegel exercises can improve control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles used to stop urination.
3. Desensitizing Products: Over-the-counter creams or sprays that contain numbing agents can reduce sensitivity and help delay ejaculation. However, it is essential to use these products as directed and consult with a healthcare provider if you have concerns.
4. Counseling or Therapy: If psychological factors are contributing to your PE, speaking with a therapist or counselor can be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and improve sexual confidence.
5. Medication: In some cases, doctors may prescribe medications such as selective serotonin reuptake inhibitors (SSRIs) or topical anesthetics to help manage PE. It is crucial to consult with a healthcare provider to discuss the best options for your situation.
6. Lifestyle Changes: Incorporating regular exercise, maintaining a healthy diet, and reducing stress through relaxation techniques can improve overall sexual health. Engaging in physical activity can enhance blood flow and improve stamina, which may help with sexual performance.
Conclusion
Premature ejaculation is a manageable condition, and there are various strategies to help improve control over ejaculation. Since you mentioned that your erection function is normal, focusing on the psychological and behavioral aspects may yield positive results. Additionally, addressing any underlying issues, such as the itching sensation, is essential. It is advisable to consult with a healthcare provider, such as a urologist or a sexual health specialist, to discuss your symptoms in detail and explore appropriate treatment options tailored to your needs. Remember, open communication with your partner about your experiences can also foster understanding and support as you work through this issue together.
