Pain upon pressure on the foreskin?
Hello, doctor.
I had unprotected oral sex 20 days ago.
At the time, I didn't feel anything, but when I got home and cleaned myself, I noticed pain in my foreskin and found a small wound.
I have a history of genital herpes and thought I would recover.
However, after twenty days, the wound is no longer visible, but there is still pain when I press on a certain area of the foreskin.
I have seen five general practitioners.
Some said it was eczema (and prescribed ointment) and others prescribed antibiotic ointment.
I also took antibiotics (ciprofloxacin 500mg) for five days.
Now, there are red spots on the glans.
I saw another doctor who suggested it might be a yeast infection.
After applying the ointment, it seems less red, but there is still pain when I press on the foreskin.
Please help me.
Thank you.

The symptoms you described are more likely indicative of balanoposthitis.
Generally, topical medication can improve the condition, but if it recurs frequently, circumcision may be considered.

More Info
Persistent pain in the foreskin can be a distressing condition, especially following a recent sexual encounter. Given your history of genital herpes and the symptoms you are experiencing, it is essential to consider several potential causes and treatment options.
Possible Causes of Persistent Pain in the Foreskin
1. Post-Inflammatory Hyperpigmentation: After an injury or infection, the skin can become sensitive and painful even after the visible signs have healed. This condition can lead to discomfort when pressure is applied.
2. Yeast Infection: As you mentioned, a yeast infection could be a possibility, especially if you have been experiencing redness and discomfort. Yeast infections can cause inflammation and pain in the genital area, and they often require antifungal treatment.
3. Balanitis: This is an inflammation of the glans (the head of the penis) and can occur due to various reasons, including infections, irritants, or allergies. Symptoms include redness, swelling, and pain, particularly when pressure is applied.
4. Dermatitis or Eczema: Skin conditions such as eczema or contact dermatitis can cause irritation and pain. If you have a history of skin sensitivities, this could be a contributing factor.
5. Scar Tissue Formation: If there was a significant injury to the foreskin, scar tissue may have formed, leading to persistent pain. Scar tissue can be less flexible and more sensitive than normal skin.
6. Nerve Sensitivity: Sometimes, after an injury or infection, the nerves in the area can become hypersensitive, leading to pain even in the absence of a physical injury.
Recommended Solutions
1. Follow-Up with a Specialist: Since you have seen multiple general practitioners without resolution, it may be beneficial to consult a urologist or a dermatologist who specializes in genital conditions. They can perform a more thorough examination and possibly conduct tests to determine the underlying cause of your pain.
2. Topical Treatments: If a yeast infection is confirmed, antifungal creams can be effective. Additionally, if dermatitis is suspected, a topical corticosteroid may help reduce inflammation.
3. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage discomfort. However, it is essential to use these as directed and consult with a healthcare provider if you have any concerns.
4. Avoid Irritants: Be mindful of any products that may irritate the area, such as scented soaps, lotions, or lubricants. Opt for gentle, unscented products to minimize irritation.
5. Hygiene Practices: Maintain good hygiene by gently cleaning the area with mild soap and water. Avoid excessive scrubbing, which can exacerbate irritation.
6. Monitor Symptoms: Keep track of any changes in your symptoms. If the pain persists or worsens, or if you develop new symptoms (such as discharge, fever, or increased redness), seek medical attention promptly.
7. Consider Psychological Factors: Sometimes, anxiety and stress can exacerbate physical symptoms. If you find that your pain is affecting your mental well-being, consider speaking with a mental health professional.
Conclusion
Persistent pain in the foreskin can arise from various causes, and it is crucial to identify the underlying issue to determine the appropriate treatment. Given your complex medical history, including genital herpes, it is advisable to seek specialized care for a thorough evaluation. With the right approach, you can find relief and address any underlying conditions contributing to your discomfort.
