Nasal bleeding

Hello Doctor: I have a question to ask you.
My mother has been experiencing nosebleeds every morning upon waking up for an unknown period of time.
She visited a clinic, but the ENT specialist did not find any signs of broken capillaries in her nasal cavity.
A CT scan was also performed, but it did not yield any diagnosis.
Until now, my mother has been coughing up sputum with blood streaks every morning.
Do you have any treatment options for this condition? Regards, A-Zhi.

A-Zhi, 10~19 year old female. Ask Date: 2003/02/04

Dr. Xu Jiazhen reply Otolaryngology

Hello, in response to your inquiry regarding coughing up blood-tinged sputum, this may indicate a pulmonary issue.
Please visit a pulmonologist for further evaluation.
As for the nasal bleeding, please follow up at the outpatient clinic to rule out any nasal tumors or other concerns.
Thank you!

Morning nosebleeds can be a concerning symptom, especially when they occur consistently upon waking. Understanding the potential causes and treatment options is crucial for managing this condition effectively.

Possible Causes of Morning Nosebleeds
1. Dry Air: One of the most common causes of nosebleeds, particularly in the morning, is dry air. This can be exacerbated during winter months or in arid climates, where humidity levels are low. Dry air can dry out the nasal mucosa, leading to cracks and bleeding.

2. Nasal Irritation: Allergies, sinus infections, or irritants (like smoke or strong odors) can cause inflammation in the nasal passages, making them more susceptible to bleeding.

3. Nasal Polyps or Structural Issues: While your mother’s previous examinations did not reveal any microvascular ruptures, other structural issues such as nasal polyps or a deviated septum could contribute to recurrent nosebleeds.

4. Medications: Certain medications, particularly blood thinners or anti-inflammatory drugs, can increase the likelihood of bleeding. If your mother is on any medication, it’s worth discussing with her healthcare provider.

5. Underlying Health Conditions: Conditions such as hypertension, blood clotting disorders, or liver disease can also lead to increased bleeding tendencies. If there are other symptoms present, it may be worth investigating these further.

6. Infections: Chronic infections, such as sinusitis, can lead to persistent nasal bleeding due to inflammation and irritation of the nasal passages.

Treatment Options
1. Humidification: Using a humidifier in the bedroom can help maintain moisture in the air, which may alleviate dryness in the nasal passages. This is especially beneficial during winter months or in dry climates.

2. Nasal Saline Sprays: Regular use of saline nasal sprays can help keep the nasal mucosa moist and reduce the likelihood of bleeding. These sprays can be used several times a day as needed.

3. Avoiding Irritants: Identifying and avoiding potential irritants, such as smoke or strong perfumes, can help reduce nasal inflammation and bleeding.

4. Proper Nose Blowing Technique: If your mother tends to blow her nose frequently, it’s important to do so gently. Forceful blowing can exacerbate bleeding.

5. Medical Evaluation: Since your mother has been experiencing these symptoms consistently, it may be beneficial to seek a second opinion from an ENT specialist. They may perform further evaluations, such as a nasal endoscopy, to visualize the nasal passages more clearly.

6. Blood Work: If there is a concern about underlying health conditions, a complete blood count (CBC) and other relevant tests can help assess for clotting disorders or other systemic issues.

7. Medication Review: If your mother is taking medications, a review with her healthcare provider can help determine if any of them may be contributing to her symptoms.

8. Lifestyle Modifications: Encouraging a healthy lifestyle, including proper hydration, a balanced diet, and regular exercise, can improve overall health and potentially reduce symptoms.

Conclusion
In summary, morning nosebleeds can arise from various causes, ranging from environmental factors to underlying health conditions. It’s essential to address the symptoms holistically, considering both immediate management strategies and potential underlying issues. If the symptoms persist or worsen, further evaluation by a healthcare professional is warranted to ensure appropriate treatment and management. Your mother’s health and comfort are paramount, and seeking the right care can lead to effective solutions.
