Is there a high chance of pregnancy if menstruation is missed?

Hello, doctor.
My last menstrual period started on December 23 and ended on December 28.
My period was expected to arrive on January 23, but as of today, January 25, it has not appeared.
I had unprotected intercourse on January 1, January 2, and January 16, but we used the withdrawal method.
In the past few weeks, I have noticed an increased frequency of urination and often feel the urge to urinate.
I also find that I feel very full after eating just a little, but shortly after, my stomach starts making noises and I feel very hungry again.
In the mornings, I sometimes feel nauseous.
I'm unsure if these symptoms are due to my anxiety or if I might be pregnant.
I am currently using contraception, and I'm really worried about not having my period this month.
Doctor, what do you think the chances of pregnancy are? Thank you for your help.

Finn, 20~29 year old female. Ask Date: 2021/01/26

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the questioner: There is a possibility of pregnancy even if ejaculation has not occurred, as pre-ejaculatory fluid may contain sperm.
Additionally, early pregnancy does not always present specific symptoms, and subjective symptoms cannot be used to make a determination.
It is recommended to check for pregnancy if your period is more than a week late.
Thank you! Wishing you peace and health.

More Info

Based on the information you've provided, it seems you are experiencing a missed period and are concerned about the possibility of pregnancy. Let's break down your situation and address your concerns.

You mentioned that your last menstrual period started on December 23 and ended on December 28. Your expected period for January was around January 23, but as of January 25, it has not yet arrived. You also noted that you had unprotected sexual intercourse on January 1, January 2, and January 16, but you used the withdrawal method (pulling out) during these encounters.
The withdrawal method is not a highly reliable form of contraception, as pre-ejaculate fluid can contain sperm, and there is always a risk of pregnancy with any unprotected sexual activity. However, if you have been consistent with your birth control methods otherwise, the likelihood of pregnancy may be lower.

You are also experiencing several symptoms that could be related to early pregnancy or anxiety. These include:
1. Increased Urination: Frequent urination can be a sign of pregnancy due to hormonal changes, but it can also be caused by anxiety or increased fluid intake.

2. Changes in Appetite: Feeling full quickly and experiencing hunger shortly after eating can be related to hormonal fluctuations, which are common in early pregnancy, but they can also be influenced by stress and anxiety.

3. Nausea: Morning sickness is a well-known symptom of pregnancy, but it can also be caused by anxiety, dietary changes, or gastrointestinal issues.

Given that you are experiencing these symptoms alongside a missed period, it is understandable that you are feeling anxious about the possibility of being pregnant.
To determine whether you are pregnant, the most reliable method is to take a home pregnancy test. These tests are designed to detect the hormone hCG (human chorionic gonadotropin), which is produced during pregnancy. It is best to take the test after your missed period for the most accurate result. If the test is positive, you should schedule an appointment with your healthcare provider for further evaluation and prenatal care. If the test is negative and your period still does not arrive, it may be wise to consult with your doctor to explore other potential causes for your missed period, such as hormonal imbalances, stress, or other medical conditions.

In conclusion, while there is a possibility of pregnancy given your sexual history, the symptoms you are experiencing could also be attributed to anxiety or other non-pregnancy-related factors. Taking a pregnancy test will provide clarity, and if you have further concerns, discussing them with your healthcare provider will help you navigate your situation more effectively. Remember that stress and anxiety can significantly impact your menstrual cycle, so managing these feelings is also important for your overall health.
