Pearly penile papules
Hello doctor, I recently noticed some white spots on my foreskin that look like pimples.
They become more pronounced when I stretch the skin, but I can't squeeze anything out.
Additionally, some areas of the foreskin are particularly red, and there are about ten white spots clustered together.
What could this possibly indicate? I haven't engaged in any sexual activity, and I'm a bit worried.
Thank you, doctor.

It could possibly be "Friedreich's ataxia spots," but it is recommended that you consult a physician for an examination.

Understanding white spots on the foreskin can be concerning, especially when they appear suddenly or change in appearance. Based on your description, it sounds like you are experiencing a condition that may have several potential causes. It's important to understand that while some conditions are benign, others may require medical attention.
One common cause of white spots on the foreskin is Pearly Penile Papules. These are small, benign growths that are often found around the corona of the glans (the head of the penis). They are typically white or skin-colored and are not associated with any pain or discomfort. Pearly penile papules are a normal anatomical variation and do not require treatment.
Another possibility is Fordyce Spots, which are small, painless, pale bumps that can appear on the shaft of the penis or the foreskin. These are sebaceous glands that are visible due to their location and are also harmless.
However, the presence of white spots accompanied by redness could indicate a condition such as Balanitis, which is inflammation of the glans and can be caused by poor hygiene, infections, or irritants. Symptoms may include redness, swelling, and sometimes discharge. Balanitis can be treated with proper hygiene and topical medications, but it’s essential to identify the underlying cause.
Another concern could be Lichen Sclerosus, a chronic inflammatory skin condition that can affect the genital area. It may present as white patches or spots and can cause itching or discomfort. This condition requires medical evaluation and may need treatment with topical steroids.
In rare cases, sexually transmitted infections (STIs) can also present with white spots or lesions, even in individuals who have not engaged in sexual activity. Conditions such as genital warts (caused by the human papillomavirus) or herpes can manifest in this way.
Given that you mentioned the spots are clustered and that there is some redness, it would be prudent to consult a healthcare provider for a thorough examination. They may perform a physical examination and, if necessary, additional tests to determine the exact cause of your symptoms.
It's important not to self-diagnose or ignore these symptoms, as early intervention can prevent potential complications. In the meantime, maintaining good hygiene and avoiding irritants (such as harsh soaps or lotions) can help alleviate some discomfort.
In summary, while there are several benign explanations for the white spots on your foreskin, the accompanying redness and clustering of the spots warrant a professional evaluation. A healthcare provider can provide a definitive diagnosis and appropriate treatment options tailored to your specific condition. Don't hesitate to reach out for medical advice, as your health and peace of mind are paramount.
