Penile issues

Hello, Dr.
Hsu: I have noticed that my penis seems to have a torsion phenomenon.
When erect, I observe that the urethral opening has rotated approximately 30 degrees counterclockwise (toward the left) when viewed from above.
I do not experience any urinary obstruction when not erect, and there is no pain during erection; I can also masturbate normally.
Having been single for many years, I have not had the opportunity to confirm whether I can engage in normal sexual intercourse.
I would like to ask the doctor if this situation is normal or if there is a need for corrective surgery.
The following link contains images taken from above while in an erect state: https://drive.google.com/file/d/1P1I_b87cEWIfdhiheSjdC36YpXFFXCFJ/view?usp=sharing.
I hope the doctor can provide guidance.
Thank you very much.

The physician should evaluate the condition, and correction is only necessary if it is severe.

Penile torsion is a condition where the penis is twisted along its shaft, which can occur during erection. In your case, you have noted a 30-degree counterclockwise twist of the urethral opening when erect, but you do not experience any pain or urinary obstruction during normal or erect states. This situation raises some important considerations regarding penile torsion and when to seek medical advice.

Firstly, it is essential to understand that a certain degree of penile torsion can be normal and may not necessarily require surgical intervention, especially if it does not cause pain, discomfort, or functional issues during sexual activity. Many men may have slight variations in penile orientation that do not affect their sexual health or quality of life. However, the degree of torsion you describe (30 degrees) is significant enough to warrant further evaluation.

Here are some key points to consider regarding your situation:
1. Assessment of Symptoms: Since you do not experience pain during erection or while engaging in sexual activities such as masturbation, this is a positive sign. However, the absence of pain does not rule out potential complications or the need for correction. It is essential to monitor any changes in symptoms, such as the onset of pain, discomfort during erections, or difficulties during sexual intercourse.

2. Potential Complications: While you currently do not have any issues, significant penile torsion can lead to complications over time. These may include erectile dysfunction, pain during intercourse, or psychological distress related to body image or sexual performance. If you anticipate engaging in sexual intercourse in the future, it may be beneficial to address the torsion beforehand to prevent any potential issues.

3. Consultation with a Specialist: It is advisable to consult a urologist who specializes in male sexual health and penile conditions. A thorough examination, including a physical assessment and possibly imaging studies, can help determine the severity of the torsion and whether surgical correction is necessary. The urologist can also discuss the potential risks and benefits of surgery if indicated.

4. Surgical Options: If surgical intervention is deemed necessary, the procedure typically involves correcting the torsion by repositioning the penis and securing it in place. This can help prevent future complications and improve sexual function. The decision for surgery should be made collaboratively with your healthcare provider, considering your personal circumstances and preferences.

5. Psychological Considerations: It is also important to consider the psychological impact of penile torsion. If you have concerns about your sexual health or body image, discussing these with a mental health professional or a sexual health counselor may be beneficial. They can provide support and strategies to cope with any anxiety or distress related to your condition.

In summary, while your current situation may not require immediate intervention, it is crucial to seek a professional evaluation to assess the degree of torsion and discuss potential treatment options. A urologist can provide personalized advice and help you make informed decisions about your sexual health. If you experience any changes in symptoms or have concerns about future sexual activity, do not hesitate to seek medical advice sooner rather than later.
