Should circumcision be performed?
Hello Doctor, I have had what seems to be an elongated foreskin since childhood, but it is not phimosis (I can manually retract it).
The glans does not naturally expose itself either when flaccid or erect, although I can manually retract it.
Recently, after suspected balanitis, the foreskin has become looser.
Previously, during an erection, it could barely hold the glans, but now it hardly does.
I have the following questions:
1.
Should I consider a circumcision, given that I can manually retract it for proper cleaning?
2.
I have been using the foreskin to rub against the glans for masturbation for over 20 years; how should I masturbate after circumcision (this is very concerning)?
3.
I have heard that after circumcision, the glans may become less sensitive, which would make life less enjoyable.

Dr. Du Shixi reply Urology
Hello: 1) Surgery is not recommended in medical practice; maintaining cleanliness is key.
2) Sensitivity of the glans will still be present, but one will get used to it over time.
3) It varies from person to person.
Wishing you good health.

When considering circumcision, especially in the context of concerns about phimosis or recurrent infections like balanitis, it’s essential to weigh the benefits and potential drawbacks carefully. Here’s a comprehensive overview addressing your concerns.
1. Should You Consider Circumcision?
Circumcision is often recommended for individuals who experience recurrent infections, such as balanitis, or those with phimosis, where the foreskin cannot be retracted fully over the glans. In your case, while you mentioned that you can manually retract the foreskin, the fact that it does not naturally retract during an erection and that you have experienced inflammation suggests that circumcision could be beneficial.
The procedure can reduce the risk of infections and may improve hygiene, as it eliminates the area where bacteria can accumulate. If you are experiencing discomfort or recurrent infections, discussing circumcision with a urologist or healthcare provider would be a prudent step. They can provide personalized advice based on your medical history and current condition.
2. Concerns About Sexual Function and Sensitivity Post-Circumcision
Your concern about how to manage sexual activity post-circumcision is valid. Many men worry about changes in sensation and sexual pleasure after the procedure. However, it’s important to note that while some men report decreased sensitivity, others find that their sexual experience improves after circumcision. This can be due to a variety of factors, including improved hygiene and reduced anxiety about infections.
If you are accustomed to masturbation with the foreskin, you may need to adapt your technique post-surgery. Many men find that they can still achieve pleasure through different methods, such as using lubrication or varying their hand movements. It may take some time to adjust, but many report that they can still enjoy sexual activities fully after circumcision.
3. Addressing Sensitivity Concerns
Regarding the concern that circumcision may lead to decreased sensitivity, it’s essential to understand that the glans (the head of the penis) becomes less sensitive over time due to exposure. After circumcision, the glans is exposed to air and friction, which can lead to a thickening of the skin, potentially reducing sensitivity. However, many men adapt to this change and continue to enjoy satisfying sexual experiences.
Moreover, studies have shown that while some men experience changes in sensitivity, many do not find it to be a significant issue. The overall satisfaction with sexual function often remains high post-circumcision.
4. Making an Informed Decision
Ultimately, the decision to undergo circumcision should be made after thorough discussions with a healthcare provider. They can help you understand the potential benefits and risks, as well as what to expect during recovery. It’s also helpful to express your concerns about sexual function and sensitivity so that your provider can address these specifically.
Conclusion
In summary, circumcision can be a beneficial procedure for individuals with recurrent infections or phimosis. While there may be concerns about changes in sexual function and sensitivity, many men adapt well and continue to have fulfilling sexual experiences. It’s crucial to have an open dialogue with your healthcare provider to make an informed decision that aligns with your health needs and personal preferences.
