Arrhythmia issues?

Background: Since childhood, I have experienced discomfort (palpitations, chest tightness, and numbness in the hands) after drinking coffee.
I usually exercise (jogging, hiking) and tend to be a more anxious person.
Recently, after swimming, I suddenly felt a tight pain in the left side of my chest that radiated to my left arm, prompting me to seek medical attention.
I underwent a 24-hour Holter monitor and an exercise stress test.
The doctor mentioned that the 24-hour Holter monitor showed over 100 extra beats but indicated that it was not a significant issue and that no further treatment was necessary.
The exercise stress test was normal.

Dr.
Huang, I have two questions:
1.
Is this level of arrhythmia not requiring treatment? The doctor advised me to relax and not to overexert myself during exercise.

2.
If the arrhythmia is caused by stress, are there any methods for improvement? Thank you for your guidance, Dr.
Huang.

Hello: The average heart rate of a person is about 100,000 beats per day, and an increase of over 100 beats in 24 hours is not significant, so there should not be any major issues.
It is recommended to start with lifestyle adjustments, such as managing and alleviating stress, maintaining a regular schedule with exercise, and avoiding stimulants like tobacco, alcohol, and coffee.
If symptoms persist, medication can sometimes be used for symptom control.
It is advisable to follow up with your physician to discuss further treatment options.

Understanding arrhythmia is crucial for anyone experiencing symptoms such as palpitations, chest discomfort, or anxiety related to heart health. Arrhythmia refers to any irregularity in the heart's rhythm, which can manifest as a heart that beats too quickly (tachycardia), too slowly (bradycardia), or irregularly. Given your background of experiencing discomfort after consuming coffee, alongside symptoms like palpitations and chest tightness, it’s important to delve into the potential causes, symptoms, and management strategies for arrhythmia.

Causes of Arrhythmia
1. Caffeine and Stimulants: As you mentioned, you have experienced discomfort after drinking coffee. Caffeine is a known stimulant that can increase heart rate and lead to palpitations in sensitive individuals. It can also exacerbate anxiety, which may further contribute to feelings of arrhythmia.

2. Stress and Anxiety: You noted that you are a person who tends to be easily tense. Stress and anxiety can significantly impact heart rhythm. The body’s response to stress involves the release of adrenaline, which can lead to increased heart rate and palpitations.

3. Electrolyte Imbalances: An imbalance in electrolytes such as potassium, sodium, calcium, and magnesium can affect heart rhythm. Regular exercise, like running or swimming, can sometimes lead to dehydration or electrolyte loss, especially if not properly managed.

4. Underlying Heart Conditions: While your recent tests, including a 24-hour Holter monitor and exercise ECG, showed no significant issues, it’s essential to consider that some arrhythmias can be benign, especially in younger individuals without a history of heart disease.

Symptoms of Arrhythmia
Common symptoms associated with arrhythmia include:
- Palpitations or a sensation of a racing heart
- Chest discomfort or tightness
- Dizziness or lightheadedness
- Shortness of breath
- Fatigue
In your case, the left-sided chest pain radiating to your left arm during swimming could be concerning, but given the normal results from your tests, it may be more related to anxiety or muscular strain rather than a serious cardiac issue.

Management Tips
1. Lifestyle Modifications: Since your doctor advised relaxation and moderate exercise, consider incorporating stress-reducing techniques such as yoga, meditation, or deep-breathing exercises. These can help manage anxiety and potentially reduce the frequency of palpitations.

2. Caffeine Reduction: If you find that caffeine exacerbates your symptoms, it may be beneficial to limit or eliminate coffee and other caffeinated beverages from your diet.

3. Regular Exercise: Continue with your exercise routine, but listen to your body. If you feel discomfort, it’s important to take a break and not push yourself too hard. Gradually increasing your activity level can help your body adapt.

4. Hydration and Nutrition: Ensure you are well-hydrated and maintain a balanced diet rich in electrolytes. Foods such as bananas (potassium), dairy (calcium), and nuts (magnesium) can support heart health.

5. Monitoring Symptoms: Keep a diary of your symptoms, noting when they occur and any potential triggers. This can help you and your healthcare provider identify patterns and adjust your management plan accordingly.

6. Follow-Up with Healthcare Providers: Regular check-ups with your healthcare provider are essential. If your symptoms persist or worsen, further evaluation may be warranted, including potential referral to a cardiologist or electrophysiologist.

Conclusion
In summary, while your current level of arrhythmia may not require immediate intervention, it is essential to manage contributing factors such as stress and caffeine intake. By adopting a proactive approach to your lifestyle and maintaining open communication with your healthcare provider, you can effectively manage your symptoms and improve your overall well-being. If you experience any new or worsening symptoms, do not hesitate to seek medical advice.
