Erectile dysfunction
Hello doctor, I have a question about the middle school student I mentioned last time.
Since around December of last year until today, I have been experiencing issues with achieving an erection, or when I do, the firmness seems inadequate, almost level with the floor.
This has happened about four times, and this is the fourth occurrence.
I do have morning erections, but they are infrequent, and sometimes there is only partial engorgement or none at all.
I am currently in the third year of junior high.
Previously, I used to masturbate almost daily, starting from sixth grade until around the second semester of junior high, when it became less frequent.
Now, I am facing these issues.
1.
Am I experiencing erectile dysfunction?
2.
What should I do if I feel anxious after masturbating?
3.
Is it easier to ejaculate when I grip the head of the penis halfway? Because previously, when I did that, I lasted longer, but after an episode of urethritis (without infection), I stopped for a few days, and my stamina has not returned to what it was; I now ejaculate more quickly.
I can last about a minute without ejaculating; does this mean I have premature ejaculation?
4.
Why do I experience recurring issues with not being able to achieve an erection or having insufficient firmness (similar to the level of the floor), while at other times everything seems normal? Is this a psychological factor? (I noticed that if I don’t think about it and relax, things improve, but if I suddenly think about it, the issue returns.)
5.
Why is it easier to achieve an erection while wearing pants, but I need to use my hand when I take them off?
I apologize for having so many questions, but I appreciate your patience in reading through them.
Thank you, doctor.

If erectile issues are intermittent but persistent, it is advisable to consult a urologist for an examination, which may include blood tests to check for hormonal imbalances.
Regarding the issue of premature ejaculation, it can vary significantly with changes in emotions and mood.
In urology, there is a concern about the possibility of prostatitis if the situation continues.
Therefore, it is important to see a urologist for further evaluation.
Additionally, the question of why it is easier to achieve an erection while wearing pants but requires manual stimulation when undressed is something I find difficult to understand.

Understanding erectile issues in adolescents can be complex, as they often involve a mix of physiological, psychological, and social factors. Your concerns are valid, and it's important to approach them with care and understanding. Let's break down your questions and provide some clarity.
1. Am I experiencing erectile dysfunction (ED)?
Erectile dysfunction is typically defined as the inability to achieve or maintain an erection sufficient for satisfactory sexual performance. In adolescents, occasional difficulties with erections can be common and may not necessarily indicate a chronic issue. Factors such as stress, anxiety, hormonal changes, and even lifestyle choices (like excessive masturbation) can temporarily affect erectile function. Given that you have experienced this issue intermittently, it may not be classified as ED, but it is worth monitoring.
2. What if I feel anxious after masturbation?
Experiencing anxiety after masturbation is not uncommon, especially in adolescents who may feel guilt or shame due to societal or personal beliefs about sexuality. It's important to recognize that masturbation is a normal part of sexual development. If the anxiety persists, consider exploring relaxation techniques or speaking with a trusted adult or counselor who can help you process these feelings.
3. Is it easier to ejaculate when gripping the penis halfway?
The way you grip during masturbation can influence the sensation and the speed of ejaculation. Different techniques can lead to varying levels of stimulation, which may affect how quickly you reach orgasm. If you find that certain methods lead to quicker ejaculation, it might be helpful to experiment with different techniques to see what works best for you. However, if you are concerned about premature ejaculation, it may be beneficial to consult a healthcare professional for tailored advice.
4. Why do I experience fluctuating erectile function?
Fluctuations in erectile function can often be attributed to psychological factors, particularly anxiety and stress. Performance anxiety, especially in sexual contexts, can lead to difficulties in achieving or maintaining an erection. It's encouraging that you've noticed a correlation between relaxation and improved function. Practicing mindfulness or relaxation techniques can be beneficial in managing anxiety and improving sexual performance.
5. Why do I find it easier to get an erection while clothed?
The sensation of clothing can sometimes provide a different type of stimulation that may lead to arousal. Additionally, the psychological aspect of being partially clothed can create a sense of excitement or anticipation. When undressed, the absence of that stimulation might lead to a different response. This is a normal variation in sexual arousal and response.
Additional Considerations:
- Hormonal Changes: As you are in your teenage years, your body is undergoing significant hormonal changes that can affect sexual function. Testosterone levels fluctuate during this time, which can impact libido and erectile function.
- Physical Health: Ensure that you maintain a healthy lifestyle, including regular exercise, a balanced diet, and adequate sleep. Physical health can significantly influence sexual health.
- Communication: If you feel comfortable, discussing these issues with a healthcare provider can provide you with personalized insights and reassurance. They can help determine if there are any underlying health concerns that need to be addressed.
In conclusion, while occasional erectile difficulties can be concerning, they are often a normal part of adolescent development. Monitoring your experiences, practicing relaxation techniques, and seeking professional guidance when necessary can help you navigate these challenges. Remember, you are not alone in this, and many adolescents face similar issues.
