Postoperative Wound Care for Fistula Surgery

Hello, doctor.
I am 20 days post-operative from a fistula incision.
A few days ago, during my follow-up appointment, the doctor said that the wound is healing well and that I do not need to return for further visits.
However, I am currently experiencing swelling, pain, and some bleeding around the wound and the anal area.
Is this normal? The original doctor prescribed an antibiotic ointment; should I continue using it? I also have itching around the anus.
Should I still use gauze pads? I have been consistently doing sitz baths with povidone-iodine in the morning, afternoon, evening, and before bed, but the surgeon mentioned that if there is no pain, sitz baths are not necessary.
I am concerned about the risk of infection or recurrence of the issue, and I would like to hear Dr.
Ke's opinion on how long I should continue the sitz baths.
Thank you!

Dr. Ke Fangxu reply Surgery

The wound from anal fistula surgery typically takes about 4-6 weeks to heal completely.
If it is a complex fistula, the healing time may extend up to two months.
During this period, slight bleeding and discharge from the wound are normal.
Antibiotic ointments are not necessary, and sitz baths can be maintained until the bleeding and discharge cease.
Remember not to use too much povidone-iodine to avoid excessive irritation that could affect wound healing.

Post-surgery care for a fistula, especially after a surgical procedure like a fistula incision, requires careful attention to pain management, bleeding control, and hygiene practices. Given that you are 20 days post-operation and still experiencing swelling, pain, and some bleeding, it is essential to understand what is typical and what may require further medical attention.

Pain and Swelling
It is not uncommon to experience some pain and swelling in the area around the surgical site for several weeks after the procedure. However, if the pain is severe or worsening, or if the swelling does not gradually decrease, it may indicate a complication such as infection or improper healing. Over-the-counter pain relief medications, such as acetaminophen or ibuprofen, can help manage discomfort, but it is crucial to follow the dosage instructions or consult your healthcare provider for recommendations tailored to your situation.

Bleeding
Mild bleeding or oozing from the surgical site can occur as part of the healing process. However, if you notice significant bleeding or if the bleeding does not stop with gentle pressure, you should seek medical attention. It is essential to monitor the amount and color of the discharge; bright red blood may indicate a more serious issue than darker, more serous fluid.

Antibiotic Ointment
If your doctor prescribed an antibiotic ointment, it is generally advisable to continue using it as directed until the prescribed course is completed or until your doctor advises otherwise. This ointment can help prevent infection and promote healing. If you notice any signs of an allergic reaction or if the area appears worse after using the ointment, contact your healthcare provider.

Hygiene and Dressing
Maintaining proper hygiene is crucial in preventing infection. You mentioned using iodine sitz baths, which can be beneficial for cleaning the area and promoting healing. While your doctor suggested that sitz baths are unnecessary if there is no pain, they can still be a soothing practice. If you feel more comfortable continuing them, it is generally safe to do so. You may want to limit the sitz baths to once or twice a day to avoid excessive moisture, which can lead to skin irritation.

Regarding the use of gauze or pads, if you are experiencing drainage, it may be wise to continue using them to keep the area clean and dry. Change the dressing regularly, especially if it becomes wet or soiled, to minimize the risk of infection.

Itching
Itching around the surgical site can be a normal part of the healing process as the skin regenerates. However, if the itching is severe or accompanied by redness, swelling, or discharge, it may indicate an allergic reaction or infection. Over-the-counter antihistamines may help alleviate itching, but consult your doctor before starting any new medication.

Follow-Up Care
While your doctor indicated that you do not need to return for follow-up, if your symptoms persist or worsen, it is essential to reach out for further evaluation. Healing can vary significantly from person to person, and your comfort and peace of mind are paramount. If you have concerns about the healing process, do not hesitate to seek a second opinion or consult with a healthcare professional who specializes in post-operative care.

In summary, while some pain, swelling, and minor bleeding can be expected after a fistula surgery, it is crucial to monitor your symptoms closely. Continue using prescribed medications, maintain good hygiene, and do not hesitate to seek medical advice if you have any concerns about your recovery. Your health and well-being should always come first.
