Nasal issues?

Hello, doctor.
I have been experiencing nasal congestion for almost a year (usually just one side is congested, but since I can still breathe normally, I haven't paid much attention to it).
However, in the past few months, I have frequently been smelling smoke, even though no one in my home smokes.
Sometimes I can smell it for several days in a row.
Could you please tell me what this might be? Thank you.

Dr. Ji Zhaoyun reply Otolaryngology

Hello, possible conditions include allergic rhinitis, sinusitis, olfactory dysfunction, nasal foreign bodies, and nasal tumors.
Common symptoms of sinusitis may include nasal congestion, yellow-green nasal discharge, fever, facial pain, foul smell, and changes in smell, although not all symptoms may be present.
If symptoms persist, it is recommended to consult an otolaryngologist for evaluation, which may include endoscopy and imaging studies as needed.
Wishing you safety and good health.

Dear Patient,
Thank you for reaching out with your concerns regarding persistent nasal congestion and the phantom smells you are experiencing. Based on your description, it seems you are dealing with a condition known as "phantosmia," which is characterized by the perception of smells that are not present in the environment. This can be particularly distressing, especially when the smells are unpleasant, such as smoke in your case.

Phantosmia can occur for various reasons, and it is often associated with issues in the olfactory system, which includes the nasal passages and the brain's processing centers for smell. Given your history of nasal congestion for nearly a year, it is possible that underlying nasal conditions, such as chronic rhinosinusitis or nasal polyps, could be contributing to your symptoms. Chronic nasal congestion can lead to inflammation and changes in the olfactory epithelium, which may result in altered smell perception.

Additionally, viral infections, such as the flu or COVID-19, can lead to temporary or even long-lasting changes in smell. In some cases, these changes can manifest as phantosmia. Other potential causes of phantosmia include neurological conditions, such as migraines or even more serious issues like temporal lobe seizures or brain tumors, although these are less common.

It is also worth considering environmental factors. If you are exposed to certain chemicals or pollutants, they could potentially trigger olfactory hallucinations. For instance, exposure to strong odors from cleaning products, paint, or even certain foods can sometimes lead to altered smell perception.

To address your concerns, I recommend the following steps:
1. Consult an ENT Specialist: Since you have been experiencing nasal congestion for an extended period, it would be beneficial to see an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination, including nasal endoscopy, to assess for any structural abnormalities, such as nasal polyps or chronic sinusitis.

2. Imaging Studies: Depending on the findings from your ENT evaluation, imaging studies such as a CT scan of the sinuses may be warranted to visualize any underlying issues that could be contributing to your symptoms.

3. Neurological Evaluation: If the ENT specialist rules out nasal causes, a referral to a neurologist may be necessary to evaluate for any neurological conditions that could be causing the phantom smells.

4. Symptom Management: In the meantime, managing your nasal congestion with saline nasal sprays, nasal corticosteroids, or antihistamines (if allergies are suspected) may help alleviate some of your symptoms. Staying hydrated and using a humidifier can also be beneficial.

5. Keep a Symptom Diary: Documenting when the phantom smells occur, their intensity, and any associated symptoms (like headaches or nasal congestion) can provide valuable information for your healthcare providers.

In conclusion, while phantosmia can be concerning, it is often manageable once the underlying cause is identified. I encourage you to seek medical attention promptly to get a clearer understanding of your symptoms and to explore appropriate treatment options. Your health and well-being are paramount, and addressing these symptoms early can lead to better outcomes.
