Frequent belching?
Hello Dr.
Chen, about three months ago, I started experiencing throat swelling, a sensation of mucus, and chest tightness (which is more severe during the day and slightly alleviated after exercise or upon waking).
I visited an ENT specialist who diagnosed me with gastroesophageal reflux disease (GERD), but I do not have any discomfort in my stomach (I occasionally burp while swimming at night, feeling like food is coming up to my throat).
The symptoms have persisted, and aside from taking medication, I have been using throat lozenges to relieve throat discomfort.
About ten days ago, I caught a gastrointestinal cold from my child, which caused diarrhea and stomach discomfort.
Although I no longer have diarrhea and my stomach discomfort has decreased, I frequently burp whenever I drink water or eat, while the chest tightness has resolved, and the throat swelling and mucus sensation have also improved.
I would like to ask whether the frequent burping is due to GERD or the gastrointestinal cold.
Should I return for a consultation to get medication? I had an endoscopy a year ago (which showed mild inflammation); do I need to have another one? I would greatly appreciate your response to my questions.
Thank you!

It may be caused by reflux; reduce intake of acidic and sweet foods, coffee, and tea.
Avoid lying flat after a heavy meal, and medication may be necessary.
If symptoms worsen, an endoscopy may be needed for confirmation.

Understanding your symptoms of frequent belching, throat discomfort, and chest tightness can indeed be challenging, especially when considering the potential overlap between gastroesophageal reflux disease (GERD) and gastrointestinal infections like gastroenteritis.
From your description, it seems that your symptoms began with throat swelling and a thick sensation, which were later attributed to GERD by an ENT specialist. GERD typically presents with symptoms such as heartburn, regurgitation, and sometimes atypical symptoms like throat discomfort or a sensation of a lump in the throat (globus sensation). The fact that you do not experience significant gastric discomfort but have occasional belching, especially after swimming or eating, suggests that your esophagus may be sensitive to refluxed material, even if you do not feel classic heartburn.
The recent episode of gastroenteritis, characterized by diarrhea and stomach discomfort, could have exacerbated your symptoms. Gastroenteritis can lead to increased gas production and altered bowel habits, which might explain the frequent belching you are experiencing now. The belching could be a result of swallowing air (aerophagia) or increased gas production in the gastrointestinal tract due to irritation or inflammation.
Regarding your question about whether the frequent belching is due to GERD or the recent gastroenteritis, it is likely a combination of both. The irritation from GERD may have made your esophagus more sensitive, while the recent gastrointestinal infection could have altered your digestive process, leading to increased gas and belching.
As for your previous endoscopy, if it was performed a year ago and showed only mild inflammation, it may not be necessary to repeat it unless your symptoms have significantly worsened or new concerning symptoms arise. However, if you continue to experience throat discomfort or if the belching persists despite treatment, it would be prudent to consult your gastroenterologist. They may recommend further evaluation, including another endoscopy, to assess any changes in your esophagus or stomach lining.
In terms of management, continuing your current medication for GERD is important. Proton pump inhibitors (PPIs) are effective in reducing stomach acid production, which can help alleviate symptoms of GERD. If you find that your symptoms are not improving, or if you have concerns about the frequency of your belching, it may be worth discussing with your doctor the possibility of adjusting your medication or exploring additional treatments.
In summary, your symptoms of frequent belching could be attributed to both GERD and the recent gastrointestinal infection. Monitoring your symptoms and maintaining communication with your healthcare provider is essential. If symptoms persist or worsen, further evaluation may be warranted. It is always best to consult with your physician for personalized advice and treatment options tailored to your specific situation.
