Struggling with Hemorrhoids: When Surgery Isn't an Option - Surgery

Mixed nevus cannot be surgically excised?

Hello doctor, I have been diagnosed with hemorrhoids.
The doctor mentioned that they are too large for band ligation (stage two) and advised me to use hemorrhoidal suppositories to reduce their size before proceeding with the ligation surgery.
However, after using the suppositories, my hemorrhoids have neither shrunk nor enlarged.
The doctor wants me to continue using them long-term, but I am feeling a bit discouraged as I haven't seen any results.
Should I continue using the suppositories, or are there other options to address my hemorrhoid issue?

Max, 30~39 year old female. Ask Date: 2021/03/21

Dr. Ke Fangxu reply Surgery

If hemorrhoids are too large to be banded, alternative treatment options should be considered instead of insisting on banding.
Each treatment method has its own advantages and limitations.
If hemorrhoids are significantly affecting your daily life, opting for surgical excision may provide more noticeable improvement.

Dealing with hemorrhoids can be quite challenging, especially when conservative treatments do not yield the desired results. Hemorrhoids, which are swollen veins in the lower rectum and anus, can cause discomfort, pain, and bleeding. When surgery is not an immediate option, it is essential to explore other avenues for management and treatment.

In your case, it seems that your doctor has recommended the use of hemorrhoid suppositories to help reduce the size of the hemorrhoids before considering a more invasive procedure like ligation. It is not uncommon for patients to feel discouraged when they do not see immediate results from treatment. However, it is important to understand that hemorrhoids can take time to respond to conservative treatments, and patience is often required.

Here are some considerations and alternative options you might explore:
1. Continued Use of Suppositories: If your doctor has advised you to continue using the suppositories, it may be beneficial to follow their recommendation for a while longer. These medications often contain ingredients that can reduce inflammation and promote healing. It is crucial to give them enough time to work, as some patients may take several weeks to notice a significant change.

2. Dietary Modifications: One of the most effective ways to manage hemorrhoids is through dietary changes. Increasing your fiber intake can help soften stools and reduce straining during bowel movements, which can exacerbate hemorrhoids. Aim for a diet rich in fruits, vegetables, whole grains, and legumes. Additionally, staying well-hydrated by drinking plenty of water can also aid in digestion and stool formation.

3. Over-the-Counter Treatments: Besides suppositories, there are various over-the-counter creams and ointments available that can provide temporary relief from pain and itching associated with hemorrhoids. These products often contain hydrocortisone or witch hazel, which can help soothe the affected area.

4. Warm Sitz Baths: Taking warm baths can help relieve discomfort and promote blood flow to the area, which may aid in healing. A sitz bath, where you sit in a few inches of warm water, can be particularly soothing and can be done several times a day.

5. Lifestyle Changes: Regular exercise can improve bowel function and reduce the risk of constipation, which is a common contributor to hemorrhoids. Additionally, avoiding prolonged sitting or standing can help alleviate pressure on the rectal area.

6. Consultation with a Specialist: If you continue to experience no improvement or if your symptoms worsen, it may be worthwhile to seek a second opinion from a gastroenterologist or a colorectal surgeon. They can provide additional insights and may suggest alternative treatments, such as rubber band ligation, sclerotherapy, or infrared coagulation, which are less invasive than traditional surgery.

7. Mindfulness and Stress Management: Stress can exacerbate gastrointestinal issues, including hemorrhoids. Engaging in mindfulness practices, yoga, or other stress-reduction techniques may help improve your overall well-being and potentially alleviate some symptoms.

In conclusion, while it can be frustrating to deal with hemorrhoids, there are several strategies you can employ to manage your symptoms effectively. It is essential to maintain open communication with your healthcare provider and to follow their recommendations while also considering lifestyle and dietary changes that can support your treatment plan. If you feel that your current treatment is not effective, do not hesitate to discuss your concerns with your doctor to explore other options.
