Post-banding bleeding after internal hemorrhoid ligation?
Hello Doctor, I had a hemorrhoid surgery on March 8th because I wanted to avoid bleeding for a long time, so I decided to go for the surgery.
For the first 12 days post-surgery, everything was normal and there was no bleeding.
However, on the 13th night, I had a larger bowel movement and strained a bit, and suddenly I felt a familiar sensation and, indeed, I started bleeding.
It seemed like quite a bit of blood, and it was dripping down.
Two nights ago, I also had a larger bowel movement and strained a little but had no issues then.
I've been eating plenty of vegetables and fruits, and I drink about 2000 ml of water daily.
Could you please explain what might be happening? The blood is bright red and separate from the stool.

After hemorrhoid ligation, there is a possibility of bleeding as a complication.
However, if bleeding occurs after more than ten days, it may be due to the rubber band falling off.
As long as the bleeding is not persistent, it is not a concern.
If the bleeding does not improve, it is important to consult a physician, as it could be due to bleeding from the ligation site or from other hemorrhoids.

More Info
Post-surgery bleeding after hemorrhoid ligation can be concerning, especially when it occurs several days after the procedure. In your case, experiencing fresh red blood after 13 days post-operation is not uncommon, but it does warrant attention. Here’s a comprehensive overview of what you might expect and how to manage the situation.
Understanding Post-Surgery Bleeding
1. Normal Healing Process: After a hemorrhoid ligation, it is typical for the surgical site to undergo a healing process that may involve some bleeding. This bleeding can occur due to several factors, including increased pressure during bowel movements, straining, or even the consumption of certain foods that may irritate the area.
2. Fresh Red Blood: The presence of bright red blood, especially if it is separate from stool, usually indicates that the bleeding is coming from the lower part of the gastrointestinal tract, likely from the site of the ligation. This is often less concerning than darker blood, which could indicate bleeding from higher up in the digestive tract.
3. Timing of Bleeding: The fact that you experienced bleeding after a relatively uneventful recovery period of 12 days suggests that it might be related to increased activity or straining during bowel movements. It’s essential to monitor the situation closely.
What to Do Next
1. Avoid Straining: If you feel the urge to have a bowel movement, try to avoid straining. Consider using a stool softener or increasing your fiber intake to ensure that your stools remain soft and easy to pass. This will help minimize pressure on the surgical site.
2. Hydration and Diet: Continue to drink plenty of fluids and consume a diet rich in fiber, including fruits and vegetables. This will aid in preventing constipation and reduce the likelihood of straining during bowel movements.
3. Monitor Symptoms: Keep an eye on the amount of bleeding. If it is a small amount and stops quickly, it may not be a cause for alarm. However, if the bleeding is heavy, persistent, or accompanied by other symptoms such as severe pain, dizziness, or fainting, you should seek medical attention immediately.
4. Follow-Up with Your Doctor: It’s crucial to communicate with your healthcare provider about any post-operative symptoms, especially bleeding. They may want to examine you to ensure that there are no complications such as infection or issues with the ligation.
5. Rest and Recovery: Ensure you are giving your body adequate time to heal. Avoid heavy lifting or strenuous activities that could increase abdominal pressure.
Potential Complications
While most cases of post-surgery bleeding resolve without intervention, there are potential complications to be aware of:
- Infection: If the bleeding is accompanied by signs of infection (increased redness, swelling, or discharge), you should contact your doctor.
- Re-bleeding: Sometimes, the ligated hemorrhoids can re-bleed, especially if they are irritated or if there is excessive straining.
- Thrombosis: In some cases, a thrombosed hemorrhoid can occur, leading to significant pain and bleeding.
Conclusion
In summary, while post-surgery bleeding after hemorrhoid ligation can be alarming, it is not always a sign of a serious problem. By managing your diet, avoiding straining, and monitoring your symptoms closely, you can help facilitate a smoother recovery. However, do not hesitate to reach out to your healthcare provider for personalized advice and to address any concerns you may have. Your health and peace of mind are paramount during your recovery process.
