Hemorrhoid ligation surgery
Hello Doctor: I underwent a hemorrhoid ligation surgery for internal hemorrhoids on October 19.
I experienced swelling and pain near the anus after the ligation, and I woke up in pain the first night.
However, by the next morning, the pain had subsided.
I noticed an increase in the amount of blood in my stool, with fresh blood appearing on a certain section of the stool's surface.
I had two bowel movements in the morning, but there was only a small amount of light red blood on the toilet paper (no constipation).
Question 1: Is the absence of swelling and pain the next morning an indication that the rubber band has fallen off?
Question 2: Is the increased amount of blood in the stool due to a rupture of the internal hemorrhoid, or could it be caused by something else?

Hello, sir: Please return to the surgeon to confirm whether there is any detachment or if the necrosis process/reaction from the ligation procedure is normal with mild bleeding.

After undergoing hemorrhoid ligation surgery, it's common for patients to experience a range of symptoms as part of the healing process. Let's address your questions regarding the symptoms you've encountered post-surgery.
1. Pain and Discomfort Post-Surgery:
It's not unusual to experience pain or discomfort after hemorrhoid ligation, especially within the first few days. The ligation procedure involves placing a rubber band around the base of the hemorrhoid to cut off its blood supply, leading to its eventual shrinkage and fall off. The pain you experienced on the first night could be attributed to the body's response to the procedure, including inflammation and pressure in the area. If the pain subsided by the second day, it might indicate that the initial inflammation was resolving or that the rubber band had indeed fallen off, which can sometimes happen within the first few days post-procedure.
2. Increased Bleeding During Bowel Movements:
The presence of fresh blood on your stool or toilet paper can be concerning, but it’s essential to understand the potential causes. After hemorrhoid ligation, some bleeding can occur as the hemorrhoid shrinks and eventually detaches. This bleeding is typically minimal and should decrease over time. However, if you notice an increase in the volume of blood or if it persists, it could indicate a few possibilities:
- Internal Hemorrhoid Rupture: If the ligated hemorrhoid has ruptured, it may lead to increased bleeding. This is usually not a cause for alarm unless the bleeding is profuse or accompanied by severe pain.
- Anal Fissures: Sometimes, the trauma from the procedure or from passing stool can cause small tears in the anal lining, leading to bleeding. This can happen even if you are not experiencing constipation.
- Infection or Inflammation: If there is significant swelling, redness, or pain in the anal area, it could suggest an infection, which may also lead to bleeding.
Recommendations:
- Monitor Symptoms: Keep an eye on the bleeding. If it continues or worsens, or if you experience significant pain, it’s crucial to consult your healthcare provider. They may want to examine the area to rule out any complications.
- Diet and Hydration: Ensure you are consuming a high-fiber diet and staying well-hydrated to facilitate smooth bowel movements. This can help prevent straining, which can exacerbate bleeding and discomfort.
- Follow-Up Care: It’s advisable to have a follow-up appointment with your surgeon or healthcare provider to discuss your symptoms and ensure proper healing. They can provide guidance on what to expect during recovery and when to seek further medical attention.
- Pain Management: If you are still experiencing discomfort, over-the-counter pain relief medications may help, but consult your doctor before taking any new medications.
In summary, while some discomfort and bleeding can be normal after hemorrhoid ligation surgery, it’s essential to monitor your symptoms closely. If you have any concerns, don’t hesitate to reach out to your healthcare provider for personalized advice and reassurance. They can help ensure that your recovery is on track and address any complications that may arise.
