Purulent nasal discharge and nasal congestion?

Hello, doctor.
I have been experiencing white nasal discharge and nasal congestion for over a month.
Last Wednesday, I visited a clinic where the doctor suggested it might be an infection, as I had a significant amount of white nasal discharge.
I was prescribed antibiotics for three days, and my symptoms improved significantly.
However, after stopping the medication, I developed thick nasal discharge and congestion again.
Today, I went back to the doctor, who prescribed another three days of antibiotics.
I am a bit worried that there might be an issue inside my nose.
I have asked the doctor twice about the need for an endoscopic examination of my nasal passages, but the doctor has reassured me that there is no problem, just an infection.

Can a simple flashlight examination of the nose reveal any issues? Should I go to a medical center for further evaluation? I have been rinsing my nose daily, and I notice a long, thin strip of clear white nasal discharge each morning and evening.
I also have a history of acid reflux and have undergone an upper endoscopy.
Once, after accidentally eating spicy food, I started experiencing nasal discharge and congestion.
Is there a connection between my current symptoms and the acid reflux? What could be causing my nasal discharge and congestion? Thank you, Dr.
Cheng, for your guidance.

Hello: The presence of common purulent nasal discharge may be associated with chronic sinusitis or chronic rhinitis.
If there has been no significant improvement after using antibiotics, or if symptoms persist, it is advisable to visit a clinic for further evaluation to determine whether a change in medication or a referral to a hospital for more extensive examination and treatment is necessary.
Wishing you safety and health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Understanding thick nasal mucus and congestion can be quite complex, as it often involves various underlying causes. Based on your description, it seems you are experiencing persistent nasal congestion accompanied by thick white mucus, which has been ongoing for over a month. You have also mentioned that you have been prescribed antibiotics, which initially helped but did not provide a lasting solution. Let's delve into the potential causes and solutions for your condition.

Causes of Thick Nasal Mucus and Congestion
1. Infections: The most common cause of thick nasal mucus is an upper respiratory infection, such as a cold or sinusitis. These infections can lead to inflammation of the nasal passages and sinuses, resulting in increased mucus production. The mucus may change in color and consistency, often becoming thicker and more opaque.

2. Allergies: Allergic rhinitis can also cause nasal congestion and thick mucus. Allergens such as pollen, dust mites, and pet dander can trigger an inflammatory response in the nasal passages, leading to increased mucus production.

3. Chronic Sinusitis: If your symptoms persist for more than 12 weeks, you may be dealing with chronic sinusitis. This condition is characterized by inflammation of the sinuses, leading to thick nasal mucus, facial pain, and pressure.

4. Gastroesophageal Reflux Disease (GERD): You mentioned having a history of acid reflux. GERD can contribute to nasal symptoms as stomach acid can irritate the throat and nasal passages, leading to post-nasal drip and increased mucus production.

5. Environmental Factors: Dry air, pollution, and exposure to irritants can exacerbate nasal congestion and mucus production. Seasonal changes can also play a role, particularly if you are sensitive to temperature fluctuations.

Solutions and Recommendations
1. Consultation with an ENT Specialist: Given your ongoing symptoms and the fact that you have already seen a doctor twice, it may be beneficial to consult an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination, potentially including nasal endoscopy, to assess the condition of your nasal passages and sinuses more accurately.

2. Nasal Irrigation: You mentioned that you are already practicing nasal irrigation, which is an excellent way to help clear mucus and reduce congestion. Using a saline solution can help moisturize the nasal passages and thin out thick mucus.

3. Medication: Since antibiotics were prescribed, it’s important to complete the course as directed. If your symptoms do not improve, discuss with your doctor the possibility of other medications, such as nasal corticosteroids, antihistamines, or decongestants, which can help reduce inflammation and mucus production.

4. Avoid Irritants: Try to minimize exposure to known irritants, such as smoke, strong odors, and allergens. Keeping your living environment clean and using air purifiers can also help.

5. Hydration: Staying well-hydrated can thin mucus, making it easier to expel. Drinking plenty of fluids, such as water and herbal teas, can be beneficial.

6. Dietary Considerations: Since you mentioned a connection between your symptoms and spicy foods, it may be wise to avoid such foods if they trigger your symptoms. Keeping a food diary can help identify any dietary triggers.

7. Follow-Up: If your symptoms persist despite treatment, ensure you have regular follow-ups with your healthcare provider. Persistent symptoms may require further investigation, including imaging studies like a CT scan of the sinuses.

In conclusion, while thick nasal mucus and congestion can often be attributed to infections or allergies, your history of GERD and the persistence of symptoms warrant a thorough evaluation by a specialist. By addressing the underlying causes and following a comprehensive treatment plan, you can find relief from your symptoms and improve your quality of life.
