Severe itching of the scrotal skin?
Hello Director Wu: I have been experiencing itching in the scrotum for the past 20 years.
I have seen various dermatologists and tried numerous medications, but there has been no improvement.
Over time, I have been using Mentholatum to alleviate the itching at night while sleeping.
However, in the past year, I have noticed that the skin on my scrotum and the surrounding groin area has developed many folds, resembling fish scales, and the scrotum has become significantly enlarged.
I would like to know how I can treat this condition, as it has been a long-standing issue for me.
I have trouble sleeping at night due to constant scratching, and the itching occurs in both dry and cold conditions as well as in hot and humid environments.
I have taken some photos of the affected area, and I was shocked to see how my skin has changed.
The red spots are wounds from scratching, which are extremely painful and itchy.

More Info
Long-term scrotal itching can be a distressing condition, and it sounds like you've been dealing with this issue for quite some time. The symptoms you've described, including the wrinkling of the skin and the enlargement of the scrotum, suggest that there may be an underlying condition that requires more than just topical treatments.
Possible Causes of Long-Term Scrotal Itching
1. Dermatological Conditions: Conditions such as eczema, psoriasis, or fungal infections (like tinea cruris) can cause chronic itching and skin changes. The appearance of scaly, wrinkled skin could indicate a dermatological issue that needs to be properly diagnosed.
2. Allergic Reactions: Allergies to soaps, detergents, or even certain fabrics can lead to persistent itching. It's essential to consider any new products you may have introduced into your routine.
3. Infections: Bacterial or fungal infections can cause significant discomfort and itching. These infections may require specific antifungal or antibacterial treatments.
4. Hygiene Issues: Poor hygiene or excessive moisture can exacerbate itching. It's important to maintain a clean and dry environment for the genital area.
5. Skin Conditions: Conditions like lichen sclerosus or lichen planus can cause chronic itching and changes in skin texture. These conditions may require specialized treatment.
Recommended Treatments
1. Consult a Dermatologist: Given the chronic nature of your symptoms and the changes in your skin, it would be wise to see a dermatologist who specializes in skin conditions. They may perform a physical examination and possibly a skin biopsy to determine the exact cause of your symptoms.
2. Topical Treatments: Depending on the diagnosis, topical corticosteroids or antifungal creams may be prescribed. These can help reduce inflammation and itching.
3. Oral Medications: If the itching is severe, oral antihistamines may be recommended to help alleviate the symptoms, especially at night.
4. Moisturizers: Regular use of emollients can help keep the skin hydrated and reduce itching. Look for products specifically designed for sensitive skin.
5. Lifestyle Modifications: Consider wearing loose-fitting, breathable underwear made from natural fibers to reduce irritation. Also, maintaining good hygiene and keeping the area dry can help prevent exacerbation of symptoms.
6. Avoid Irritants: Identify and avoid any potential irritants, including soaps, lotions, or laundry detergents that may be causing allergic reactions.
7. Diet and Hydration: Sometimes, dietary changes can help with skin conditions. Staying well-hydrated and maintaining a balanced diet rich in vitamins and minerals can support skin health.
Conclusion
Chronic scrotal itching can significantly impact your quality of life, and it’s essential to address it properly. Since you've already tried various treatments without success, a thorough evaluation by a specialist is crucial. They can provide a tailored treatment plan based on the specific diagnosis. Remember, self-diagnosing and treating can sometimes lead to further complications, so professional guidance is key. Don't hesitate to seek help, as there are effective treatments available that can help alleviate your symptoms and improve your quality of life.
