Testicular itching
Hello Doctor: I have been experiencing itching in my testicles for 1 to 2 years.
Initially, it was just a slight itch, but over time it developed into skin breakdown and scabbing, forming a thin membrane that can be easily scraped off with my fingernail.
Now, the itching intensifies until the skin breaks.
I consulted a doctor who mentioned that my testicles are too dry, which is causing the itching.
Recently, I've also started to experience itching in my groin area.
This issue has been troubling me for 2 years.
Is there a way to treat this? What exactly is this condition? Please let me know, Doctor.

Dr. Pan Zhenqin reply Urology
Scrotal itching may be caused by dry skin, tinea, follicular issues, or allergic reactions.
It is advisable to clarify the timing of the itching in relation to the season, temperature, sexual activity, diet, or medications, and whether there are any other areas of the body experiencing similar symptoms.
Then, when the itching occurs, visit a urology or dermatology clinic to determine the possible causes.

More Info
Testicular itching can be a distressing condition, and understanding its causes and treatment options is essential for effective management. Based on your description, it seems that you have been experiencing persistent itching in the testicular area for about 1 to 2 years, which has progressed to skin irritation and peeling. This condition can be attributed to several potential causes, and I will outline them along with possible treatment options.
Causes of Testicular Itching
1. Dry Skin: As your doctor mentioned, dryness can lead to itching. The skin in the groin area is sensitive, and environmental factors such as low humidity, harsh soaps, or excessive washing can strip the skin of its natural oils, leading to dryness and irritation.
2. Fungal Infections: Conditions like tinea cruris (jock itch) are common in the groin area and can cause intense itching, redness, and flaking of the skin. This is often exacerbated by moisture and friction.
3. Contact Dermatitis: This is an allergic reaction to substances that come into contact with the skin, such as soaps, detergents, or fabrics. If you have recently changed any products, this could be a contributing factor.
4. Bacterial Infections: Sometimes, bacterial infections can occur, especially if there is broken skin. This can lead to further irritation and itching.
5. Skin Conditions: Conditions such as eczema or psoriasis can also affect the genital area, leading to itching and skin changes.
6. Hygiene Practices: Inadequate hygiene can lead to the accumulation of sweat and bacteria, which can cause itching. Conversely, excessive washing can strip the skin of its natural oils.
7. Underlying Medical Conditions: Conditions such as diabetes or sexually transmitted infections (STIs) can also manifest as itching in the genital area.
Treatment Options
1. Moisturizers: If dryness is the primary issue, using a gentle, fragrance-free moisturizer can help restore hydration to the skin. Look for products specifically designed for sensitive skin.
2. Antifungal Creams: If a fungal infection is suspected, over-the-counter antifungal creams (like clotrimazole or miconazole) can be effective. If symptoms persist, a prescription antifungal may be necessary.
3. Topical Steroids: For inflammation or allergic reactions, a mild topical steroid cream can help reduce itching and irritation. However, these should be used under the guidance of a healthcare provider.
4. Avoid Irritants: Identify and avoid any potential irritants, such as harsh soaps, laundry detergents, or tight clothing. Opt for breathable, cotton underwear to reduce moisture buildup.
5. Proper Hygiene: Maintain good hygiene by washing the area with mild soap and water, and ensure it is thoroughly dried afterward. Avoid excessive washing, which can lead to dryness.
6. Consult a Dermatologist: If the condition does not improve with over-the-counter treatments, or if you notice any unusual symptoms (such as discharge, severe redness, or swelling), it is important to consult a dermatologist or urologist for further evaluation.
7. Lifestyle Modifications: Consider lifestyle changes that can help, such as wearing loose-fitting clothing, avoiding prolonged sitting, and managing sweat through regular bathing.
Conclusion
Persistent testicular itching can be caused by a variety of factors, and it is essential to identify the underlying cause to determine the most effective treatment. Since you have been experiencing this issue for an extended period, I recommend following up with a healthcare provider who can perform a thorough examination and possibly conduct tests to rule out infections or other skin conditions. With the right approach, it is possible to manage and alleviate your symptoms effectively.
