Itchy testicles and itching at the tip of the foreskin?


Hello doctor, I have been troubled by itching on both sides and the underside of my testicles for a long time.
Sometimes it starts after masturbation, and other times it occurs at night.
Generally, I can tolerate it, but I often find myself scratching uncontrollably in my sleep.
I usually apply topical steroids like Betamethasone or use hot water to relieve the discomfort.
Recently, I've also started experiencing itching and even pain at the glans and foreskin, but I'm unsure how to address it.
