Prostatitis and dry orgasm without semen?

Hello doctor, I just had a urology appointment the day before yesterday, and the doctor diagnosed me with chronic prostatitis.
Later, I found online that an appropriate frequency of ejaculation can help with prostate metabolism.
However, today when I masturbated, I noticed a dry orgasm with no semen.
I usually do it three to four times a week, which is generally considered normal, right? I read online that excessive indulgence might lead to insufficient secretion of prostatic fluid, resulting in the inability to transport semen.
Am I causing secondary damage to my prostate? Should I see a doctor again?

Lawren, 20~29 year old female. Ask Date: 2021/03/24

It sounds like retrograde ejaculation caused by medications used to treat prostatitis.

Chronic prostatitis, particularly nonbacterial prostatitis, can be a challenging condition to manage, especially when it comes to understanding its symptoms and potential complications. Your experience with dry orgasms and concerns about the frequency of ejaculation are valid and warrant further exploration.

Firstly, it's important to clarify that chronic prostatitis can manifest in various ways, including pelvic pain, urinary symptoms, and sexual dysfunction. The relationship between ejaculation frequency and prostate health is complex. Research suggests that regular ejaculation may help reduce the risk of prostate issues, including prostatitis, by promoting the drainage of prostatic secretions and potentially reducing inflammation. However, the optimal frequency of ejaculation can vary from person to person.

In your case, experiencing dry orgasms—where ejaculation does not occur—can be concerning, especially if you are also dealing with chronic prostatitis. This phenomenon can occur for several reasons, including:
1. Prostate Inflammation: Inflammation of the prostate can affect its ability to produce and expel seminal fluid, leading to dry orgasms. This may be exacerbated by the chronic nature of your prostatitis.

2. Nerve Function: The nerves that control ejaculation may be affected by pelvic pain or inflammation, leading to difficulties in the ejaculation process.

3. Medication Effects: Certain medications, particularly those used to treat prostatitis or associated symptoms, can impact sexual function and ejaculation.

Regarding your concern about whether your sexual activity could lead to further damage to the prostate, it is essential to understand that while excessive sexual activity or "overindulgence" can lead to temporary discomfort or exacerbation of symptoms, it is unlikely to cause permanent damage. However, if you are experiencing pain or discomfort during or after sexual activity, it may be wise to adjust your frequency and consult with your healthcare provider.

If you find that your symptoms persist or worsen, or if you have ongoing concerns about dry orgasms and their implications for your prostate health, it would be advisable to follow up with your urologist. They can provide a thorough evaluation and may suggest additional treatments or lifestyle modifications to help manage your symptoms effectively.

In the meantime, consider the following recommendations:
1. Maintain a Healthy Lifestyle: Regular exercise, a balanced diet, and adequate hydration can support overall prostate health. Avoiding irritants such as caffeine, alcohol, and spicy foods may also help reduce symptoms.

2. Pelvic Floor Exercises: Engaging in pelvic floor exercises (Kegel exercises) may help strengthen the pelvic muscles and improve urinary and sexual function.

3. Stress Management: Chronic pain and prostatitis can be exacerbated by stress. Techniques such as mindfulness, meditation, or yoga may help alleviate some of the discomfort associated with chronic prostatitis.

4. Regular Follow-ups: Keep in touch with your healthcare provider to monitor your condition and adjust treatment as necessary. They may recommend physical therapy or other interventions to address pelvic pain and dysfunction.

In summary, while your concerns about ejaculation frequency and dry orgasms are valid, they should be discussed with your healthcare provider to ensure you receive personalized advice and treatment. Chronic prostatitis can be a complex condition, but with appropriate management, many individuals find relief from their symptoms and improve their quality of life.
