Perineal mass
Hello, doctor.
As stated in the title, I have noticed a lump on the right side of my perineum for many years, which I discovered while wiping after using the restroom.
It is slightly painful when pressed and feels like a foreign body, but it has not ruptured, so there is no discharge.
Recently, I have noticed that the lump has increased in size.

Based on your description, this perineal mass may be a subcutaneous cyst or a lipoma.
It is recommended to visit a surgical outpatient clinic for further examination and confirmation.

More Info
Understanding a perineal mass can be quite concerning, especially when it has been present for an extended period and has recently increased in size. The perineum is the area between the anus and the genitals, and masses in this region can arise from various causes, each with distinct characteristics, symptoms, and treatment options.
Causes of Perineal Masses
1. Cysts: The most common cause of a perineal mass is a cyst, such as a sebaceous cyst or epidermoid cyst. These are usually benign and can occur due to blocked sebaceous glands or hair follicles.
2. Abscesses: An abscess is a collection of pus that can develop due to infection. Although you mentioned that there is no discharge or signs of rupture, an abscess can sometimes be present without overt symptoms.
3. Hemorrhoids: Swollen blood vessels in the rectal area can create a mass-like feeling, especially if they become thrombosed (clotted).
4. Lipomas: These are benign tumors made of fat tissue. They are generally soft and movable under the skin.
5. Fistulas: An abnormal connection between the rectum and the skin can lead to the formation of a mass, often associated with pain and discharge.
6. Malignancies: Although less common, tumors in the perineal area can be malignant. Conditions such as squamous cell carcinoma or other types of skin cancer can present as masses.
Symptoms
In your case, the mass is described as being slightly painful upon pressure and giving a sensation of a foreign body. The absence of discharge or rupture is a positive sign, but the recent increase in size warrants further investigation. Other symptoms that may accompany a perineal mass include:
- Pain or discomfort in the area
- Changes in bowel habits
- Bleeding or discharge
- Changes in urinary function
- Swelling or redness in the surrounding area
Diagnosis
To accurately diagnose the cause of the perineal mass, a healthcare provider will typically perform a physical examination and may recommend imaging studies such as an ultrasound or MRI. In some cases, a biopsy may be necessary to rule out malignancy.
Treatment Options
1. Observation: If the mass is benign and asymptomatic, a "watchful waiting" approach may be taken.
2. Surgical Removal: If the mass is causing discomfort, increasing in size, or if there is a concern for malignancy, surgical excision may be recommended.
3. Drainage: For abscesses, drainage may be necessary to relieve pain and prevent further infection.
4. Medications: If the mass is associated with inflammation or infection, antibiotics or anti-inflammatory medications may be prescribed.
5. Follow-Up: Regular follow-up appointments may be necessary to monitor the mass, especially if it is being observed rather than treated immediately.
Conclusion
Given the details you've provided about the perineal mass, it is essential to consult a healthcare provider for a thorough evaluation. The increase in size, along with any discomfort, should not be overlooked. Early diagnosis and appropriate management are crucial in ensuring the best possible outcome. If you experience any additional symptoms such as fever, significant pain, or changes in bowel or urinary habits, seek medical attention promptly.
