The issue of egg freezing?
Hello Dr.
Tsao: I read an article stating that breast cancer patients undergoing assisted reproduction should control the dosage of ovulation induction medications and keep estrogen levels relatively low to prevent recurrence of breast cancer.
My mother and aunt have had breast cancer, and there is a breast cancer gene in my family.
I underwent a normal egg freezing procedure without controlling for lower estrogen levels.
Am I at risk of inducing breast cancer? I am very concerned.
Thank you for your response.

When it comes to concerns about egg freezing and breast cancer risk, it's essential to understand the relationship between hormonal treatments used in assisted reproductive technologies (ART) and the potential implications for individuals with a family history of breast cancer.
Firstly, it is important to note that the use of hormonal medications, such as those involved in egg freezing procedures, typically involves the administration of gonadotropins to stimulate the ovaries. These medications can lead to increased levels of estrogen, which has been a concern for individuals with a history of breast cancer or those with a family history of breast cancer, particularly if they carry mutations in the BRCA1 or BRCA2 genes.
Research indicates that elevated estrogen levels can potentially stimulate the growth of hormone-sensitive breast cancer cells. However, the actual risk of developing breast cancer as a result of undergoing egg freezing procedures is still a topic of ongoing research. Current evidence suggests that for most women, including those with a family history of breast cancer, the short-term use of hormonal medications for egg freezing does not significantly increase the risk of breast cancer recurrence or the development of new breast cancer.
For women with a family history of breast cancer, it is crucial to have a thorough discussion with a healthcare provider specializing in reproductive endocrinology and oncology. This discussion should include an assessment of personal and family medical history, genetic testing results, and the specific hormonal protocols that will be used during the egg freezing process. In some cases, doctors may recommend adjusting the hormonal regimen to minimize estrogen exposure, although this may vary based on individual circumstances.
Moreover, it is essential to consider that the decision to undergo egg freezing should also take into account other factors, such as age, fertility potential, and personal reproductive goals. For women who are concerned about their breast cancer risk, it may be beneficial to explore options such as preimplantation genetic testing (PGT) for embryos, which can help identify genetic abnormalities before implantation.
In summary, while there are valid concerns regarding the use of hormonal treatments in women with a family history of breast cancer, the current consensus is that egg freezing, when done under the guidance of knowledgeable healthcare professionals, is generally safe. Women should feel empowered to seek second opinions and gather as much information as possible to make informed decisions about their reproductive health. Regular screenings and monitoring for breast cancer, particularly for those at higher risk, remain crucial components of a comprehensive health strategy.
Ultimately, the key takeaway is that while hormonal treatments can influence breast cancer risk, the context of each individual's health history and the specifics of their treatment plan are paramount. Engaging in open dialogue with healthcare providers can help alleviate concerns and ensure that women make choices that align with their health needs and reproductive desires.
