Conception Dates: Can I Be Pregnant from March 19? - Obstetrics and Gynecology

Pregnancy date

Hello Doctor: My menstrual cycle is very regular at 28 days, with the first day of my last period on March 13 (there was light bleeding on March 11 and March 12).
I had sexual intercourse on March 1, March 8, March 15, March 19, and March 24, all without contraception, with the intercourse on March 19 being with my ex-boyfriend.
I did not get my period in April, and on April 15, I took a urine test at the obstetrics and gynecology department which confirmed pregnancy.
An ultrasound did not detect an embryo, and on April 29, the ultrasound indicated I was 7 weeks along.
Considering whether to get married and whether to have the baby, I would like to ask the doctor if the sexual intercourse on March 19 could have resulted in pregnancy.
Thank you!

Fang, 30~39 year old female. Ask Date: 2021/04/29

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello Miss Fang: Your menstrual cycle is regular, approximately 28 days, so ovulation occurs around the 14th day of your cycle.
Therefore, the five days leading up to March 26 and the two to three days following it are considered the fertile window.
This means that the period from March 21 to March 29 is the fertile window.
Consequently, the likelihood of conception on March 19 is very low.

More Info

To determine whether you could have become pregnant from the sexual encounter on March 19, we need to analyze your menstrual cycle and the timing of ovulation in relation to your sexual activity.

You mentioned that your menstrual cycle is regular, typically lasting 28 days. This means that ovulation likely occurs around day 14 of your cycle, which would be approximately March 27 in your case, given that your last menstrual period started on March 13. The fertile window, which is the period during which conception is most likely to occur, generally spans from about five days before ovulation to one day after ovulation. Therefore, for a cycle of 28 days, the fertile window would likely be from March 21 to March 29.

Now, let's look at the dates of your sexual encounters:
- March 1
- March 8
- March 15
- March 19 (with your ex-boyfriend)
- March 24
Among these dates, the sexual activity on March 19 falls just before your estimated ovulation period. While sperm can survive in the female reproductive tract for up to five days, the likelihood of conception from intercourse on March 19 is relatively low, as it is outside the peak fertile window. The highest chances of conception would be from sexual activity occurring closer to ovulation, particularly between March 21 and March 29.

You also mentioned that your period did not arrive in April, and a pregnancy test on April 15 confirmed that you were pregnant. The ultrasound on April 29 indicated that you were approximately 7 weeks along, which would place your estimated conception date around the time of ovulation, likely around March 27.

In summary, while it is theoretically possible to conceive from intercourse on March 19, the timing suggests that it is unlikely. The more probable scenario is that conception occurred from sexual activity closer to your ovulation date, which would be around March 27.
If you are considering your options regarding marriage and having a child, it is essential to consult with a healthcare provider who can provide personalized advice based on your health and circumstances. They can also help you understand the implications of your pregnancy and discuss any concerns you may have about your future.
In conclusion, while the sexual encounter on March 19 is not the most likely cause of your pregnancy, it is important to consider the timing of your ovulation and the possibility of conception from other encounters within your fertile window. If you have further questions or concerns, please feel free to reach out for more information.
