Issues with Vomiting After Taking Emergency Contraceptive Pills

Hello doctor, I am currently on my second pack of the contraceptive pill "Yueji." My last menstrual period was from April 15 to April 19, and I took one pill every day at 9 PM.
On April 30, I vomited within an hour after taking the pill due to drinking alcohol.
Today, around 3 PM, I took an additional pill and plan to take another one at 9 PM.
I would like to know if I still need to take emergency contraception.
If so, can I continue taking "Yueji" normally?

Zhen, 10~19 year old female. Ask Date: 2021/05/02

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello Miss Zhen: Since you have already taken one extra contraceptive pill and have continued to take your birth control pills at the regular time afterward, this approach is completely correct.
There is no need to worry about the possibility of pregnancy.
You do not need to take emergency contraception.
Just continue taking your regular birth control pills.

When it comes to the use of birth control pills and the potential need for emergency contraception after vomiting, it's essential to understand how these medications work and the implications of vomiting shortly after taking them.

Birth control pills, like the one you mentioned (悦己, or "Yue Ji"), are designed to prevent ovulation and thus reduce the risk of pregnancy. However, their effectiveness can be compromised if they are not taken correctly. Vomiting can interfere with the absorption of the hormone in the pill, especially if it occurs within two hours of taking the pill. In your case, since you vomited about an hour after taking the pill on April 30, there is a possibility that your body did not fully absorb the medication.

Given that you vomited shortly after taking your birth control pill, it is advisable to take another pill as soon as possible, which you did by taking an additional pill in the afternoon. This is a good step to ensure that you maintain the hormone levels necessary to prevent ovulation. However, since you are still within the same cycle and have taken the additional pill, you should continue taking your regular birth control pills as scheduled.

Now, regarding the need for emergency contraception (EC), it is generally recommended if you have had unprotected intercourse after a missed dose or if you are unsure about the effectiveness of your birth control due to vomiting. If you had unprotected sex after the vomiting incident and are concerned about the possibility of pregnancy, taking emergency contraception could be a prudent choice. EC is most effective when taken as soon as possible after unprotected intercourse, ideally within 72 hours, but some forms can be effective up to 5 days after.

You can continue taking your regular birth control pills after taking emergency contraception. However, it's important to note that emergency contraception may cause some irregularities in your menstrual cycle, and it does not provide ongoing protection against pregnancy. Therefore, it is crucial to continue taking your regular birth control pills as directed to maintain contraceptive efficacy.

In summary, if you vomited within two hours of taking your birth control pill and had unprotected sex afterward, it is wise to consider taking emergency contraception. Continue with your regular birth control regimen, and if you have any further concerns or experience any unusual symptoms, consult your healthcare provider for personalized advice.

Additionally, it is always a good practice to have a backup method of contraception, such as condoms, especially during times when you might be at risk of missed doses or absorption issues, such as vomiting or diarrhea. This can provide extra peace of mind and help prevent unintended pregnancies. If you have any further questions or concerns about your birth control method or any other reproductive health issues, do not hesitate to reach out to your healthcare provider for guidance.
