Taking birth control pills and antibiotics simultaneously?

Hello Doctor: I experienced a urinary tract infection on April 15-16 and took Keflex 500 mg (Cephalexin), Ponstan 260 mg, FOXATE F.C.
Tablets 200 mg, and Suwell 500 mg, while I was already taking the contraceptive pill Diane.
On April 18, my app indicated that it was my ovulation period, and I had unprotected intercourse that day.
Will this affect the effectiveness of the contraceptive?

Terry, 20~29 year old female. Ask Date: 2021/05/03

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the following medications may interact with oral contraceptives: 1.
Antibiotics (Penicillin, Cephalosporins, Erythromycin, Sulfonamides, Bactrim, Chloramphenicol) - may reduce the gut flora that aids in absorption, thereby decreasing the effectiveness of contraceptives.
2.
Sedatives (Barbiturates) - increase the metabolism of contraceptives, reducing their effectiveness.
3.
Antiepileptic drugs (Primidone, Phenytoin, Carbamazepine) - increase the metabolism of contraceptives, reducing their effectiveness.
4.
Antituberculosis medications (Rifampicin, Isoniazid).
5.
Antidiabetic medications (Rosiglitazone).
6.
Acetaminophen and Ascorbic acid (Vitamin C) - may inhibit the metabolism of estrogen, leading to increased serum levels and a higher incidence of side effects.
7.
Oral contraceptives may affect the action of anticoagulants.
8.
Smoking - increases the metabolism of contraceptives, reducing their effectiveness.
If there is no expected withdrawal bleeding during the placebo period after taking the pills continuously, pregnancy should be suspected.
Thank you! Wishing you peace and health, Dr.
Zhang.

When it comes to the interaction between antibiotics and birth control pills, it is essential to understand how these medications work and their potential effects on contraceptive efficacy. The concern arises primarily from the belief that certain antibiotics may reduce the effectiveness of hormonal contraceptives, such as birth control pills.

First, let's clarify the situation with the specific antibiotics mentioned in your case. Cephalexin (Keflex) is a cephalosporin antibiotic that is generally not known to interfere with the effectiveness of birth control pills. Most studies and clinical guidelines indicate that cephalosporins do not have a significant impact on hormonal contraceptives. Therefore, taking Cephalexin while on birth control pills should not compromise their effectiveness.

On the other hand, Penicillin and Tetracycline antibiotics have historically been associated with reduced effectiveness of birth control pills, but this is primarily due to their potential to affect the gut flora and, consequently, the metabolism of the hormones in the contraceptive pills. However, the evidence is not conclusive, and many healthcare providers now believe that the risk is minimal.

Regarding the other medications you mentioned, Ponstan (Mefenamic Acid) is a non-steroidal anti-inflammatory drug (NSAID) used primarily for pain relief and does not affect the efficacy of birth control pills. Foxate F.C. Tablets and Suwell are not commonly referenced in relation to birth control interactions, but they are not known to have any significant effects on hormonal contraceptives either.

Now, addressing your concern about the timing of your ovulation and the potential for pregnancy: If you took the antibiotics as prescribed and continued your birth control pills without missing any doses, the likelihood of reduced effectiveness is low. Since you mentioned that you had unprotected intercourse on your ovulation day (April 18), it is important to note that this is typically the time when the chances of conception are highest. However, if your birth control pills were taken consistently and correctly, the risk of pregnancy remains low, even with the use of antibiotics like Cephalexin.

In summary, based on current medical understanding, taking Cephalexin while on birth control pills should not significantly affect their effectiveness. However, if you have concerns about potential pregnancy or the effectiveness of your contraceptive method, it is advisable to consult with your healthcare provider. They can provide personalized advice and may suggest additional contraceptive measures, such as using condoms, for added protection during the time you are taking antibiotics or if you have any doubts about your birth control's efficacy.

Lastly, if you are ever prescribed antibiotics in the future, it is always a good practice to discuss with your healthcare provider about any potential interactions with your birth control method. They can provide the most accurate and up-to-date information tailored to your specific situation.
